ORIGINAL ARTICLE Assessment of the information quality of chatbot technologies on orthodontic miniscrews YAMAÇ, Başak Baş AKÇAR, Rojda ŞARKAN, İpek Eryılmaz ARSLAN, Can Resumo em Português: RESUMO Introdução: Nesta era da inteligência artificial, a crescente concorrência aumentou significativamente o poder dos chatbots de IA, os quais foram integrados em várias áreas, incluindo a Ortodontia. Eles podem ser uma boa opção para esclarecer temas de curiosidade antes ou durante o tratamento, pois são facilmente acessíveis pelos pacientes. No entanto, sua confiabilidade continua sendo uma preocupação. Os mini-implantes, ou dispositivos de ancoragem temporária (DATs), frequentemente usados durante os tratamentos ortodônticos, estão entre os assuntos de interesse dos pacientes. Objetivo: Este estudo teve como objetivo avaliar comparativamente as respostas fornecidas por quatro Modelos de Linguagem de Grande Escala (LLMs), ChatGPT-3.5, ChatGPT-4 (OpenAI), Google Bard (Google LLC) e Bing Chat (Microsoft Corp), a perguntas feitas por pacientes sobre o tema mini-implantes ortodônticos. Material e Métodos: As perguntas mais frequentes dos pacientes sobre mini-implantes utilizados em Ortodontia foram pesquisadas no Google. As primeiras 50 páginas foram analisadas e 30 perguntas foram selecionadas e enviadas aos LLMs. Três residentes em Ortodontia avaliaram as respostas dos LLMs utilizando uma escala modificada de cinco pontos e o DISCERN modificado (mDISCERN). Resultados: Observou-se que o escore mais alto para Likert pertencia ao GPT-4 (3,84), enquanto o mais baixo foi para o Bing Chat (3,37). Foram encontradas diferenças estatisticamente significativas entre os valores médios dos escores atribuídos às perguntas pelos três pesquisadores, dependendo dos LLMs utilizados (p = 0,016; p<0,001 e 0,017, respectivamente). Foram encontradas diferenças significativas entre os escores atribuídos pelo Investigador 1 e pelo Investigador 3 ao ChatGPT-4 e ao Google Bard, e os escores atribuídos ao Bing Chat. Além disso, o Investigador 2 apresentou diferenças significativas nos escores atribuídos ao Bing Chat, em comparação com o ChatGPT-3.5 (p=0,018). Entre os chatbots avaliados, o ChatGPT-4 obteve o escore mDISCERN mais alto (23,43 ± 2,89), seguido pelo Google Bard (23,47 ± 3,2), ChatGPT-3.5 (22,62 ± 2,97) e Bing Chat (21,63 ± 2,43). Conclusões: Neste estudo, verificou-se que os LLMs podem, em geral, informar os pacientes sobre os mini-implantes utilizados em Ortodontia e têm um potencial promissor. No entanto, é necessário que as informações fornecidas por esses programas sejam sempre complementadas pelas informações fornecidas por profissionais.Resumo em Inglês: ABSTRACT Introduction: In this era of artificial intelligence, the increasing competition has significantly enhanced the power of AI chatbots, which have been integrated into various fields, including orthodontics. They can be a good option for enlightening topics of curiosity before or during treatment, as they are easily accessible by patients. However, their reliability remains a concern. Miniscrews, or Temporary Anchorage Devices (TADs), frequently used during orthodontic treatments, are among the subjects of interest to patients. Objective: This study aimed to comparatively evaluate the responses provided by four Large Language Models (LLMs), namely ChatGPT-3.5, ChatGPT-4 (OpenAI), Google Bard (Google LLC), and Bing Chat (Microsoft Corp), to questions asked by patients in the field of orthodontic miniscrews. Material and Methods: The most frequently asked questions by patients about miniscrews used in orthodontics were searched on Google. The first 50 pages were reviewed, and 30 questions were selected and asked to the LLM. The responses from the LLMs were evaluated using a five point modified scale and modified DISCERN (mDISCERN) by three orthodontic residents. Results: It was observed that the highest score for Likert belonged to GPT-4 (3.84), while the lowest was for Bing Chat (3.37). Statistically significant differences were found between the median score values given to the questions by all three researchers, depending on the LLMs used (p = 0.016; p<0.001 and 0.017, respectively). Significant differences were found between the scores given by Investigator 1 and Investigator 3 to ChatGPT-4 and Google Bard and the scores given to Bing Chat. Additionally, Investigator 2 showed significant differences in the scores given to Bing Chat compared to ChatGPT-3.5 (p=0.018). Among the evaluated chatbots, ChatGPT-4 achieved the highest mDISCERN score (23.43 ± 2.89), followed by Google Bard (23.47 ± 3.2), ChatGPT-3.5 (22.62 ± 2.97), and Bing Chat (21.63 ± 2.43). Conclusions: In this study, it was found that LLMs can generally inform patients about miniscrews used in orthodontics and have promising potential. However, it is necessary that the information provided by these programs should always be supported by the information given by professionals. |

ORIGINAL ARTICLE Rapid maxillary expansion and its impact on sleep apnea in children aged 5 to 8 years: a retrospective study BAVANI, Sarvenaz Zaree NG, Enoch T. VICH, Manuel O Lagravère Resumo em Português: RESUMO Objetivo: Determinar o impacto da expansão transversal precoce da maxila na manifestação sintomática da apneia do sono em crianças de 5 a 8 anos. Métodos: Dezenove pacientes (idade média: 6,84 anos) com constrição maxilar foram submetidos a expansão rápida da maxila (EMR) para tratamento ortodôntico. Critérios de inclusão: idade entre 5 e 8 anos, Índice de Apneia-Hipopneia (IAH) > 1, tratamento com aparelho Hyrax com parafuso de memória, sob cuidados de médico pediatra especialista em sono. Critérios de exclusão: não adesão ao tratamento, atrasos na reavaliação (> 3 meses). Variável primária: IAH antes (T0) e após (T1) a expansão. O tratamento foi encerrado com base na correção da má oclusão. As comparações do IAH incluíram o estado da dentição e o sexo como variáveis secundárias. Resultados: O teste t pareado não mostrou diferença significativa na média do IAH entre T0 (4,05±2,55) e T1 (3,68±3,12) para as variáveis primárias (p>0,05). Em T0, não foi encontrada diferença significativa no IAH entre os sexos (F: 3,21 ± 2,47; M: 4,09 ± 2,87, p>0,05). Após a expansão (T1), observou-se uma mudança significativa entre os sexos (F: 1,75±1,03; M: 5,18±3,68, p<0,05). Também não houve diferença significativa nas mudanças do IAH com base no status da dentição. Foi encontrada uma correlação negativa entre as mudanças do IAH e as mudanças transversais e nos níveis dos caninos e molares. Conclusão: Não foram encontradas evidências significativas que comprovem a eficácia da expansão maxilar precoce na melhoria da apneia do sono em crianças com constrição maxilar. No entanto, as respostas específicas de cada sexo e a potencial relação dose-resposta entre a expansão maxilar e a redução do IAH destacam a complexidade do tratamento da AOS pediátrica.Resumo em Inglês: ABSTRACT Objective: Determine the impact of early transverse maxillary expansion on the symptomatic manifestation of sleep apnea in children aged 5-8 years. Methods: Nineteen patients (mean age: 6.84 years) with maxillary constriction underwent rapid maxillary expansion (RME) for orthodontic treatment. Inclusion criteria: age 5-8 years, Apnea-Hypopnea Index (AHI) > 1, treated with Memory screw Hyrax appliance, under pediatric sleep physician care. Exclusion criteria: non-compliance, re-evaluation delays (> 3 months). Primary variable: AHI before (T0) and after (T1) expansion. Treatment ended based on malocclusion correction. AHI comparisons included dentition status and gender as secondary variables. Results: A paired t-test showed no significant difference in the mean AHI between T0 (4.05±2.55) and T1 (3.68±3.12) for primary variables (p>0.05). At T0, no significant AHI difference was found between genders (F: 3.21±2.47; M: 4.09±2.87, p>0.05). After expansion (T1), a significant change was observed between genders (F: 1.75±1.03; M: 5.18±3.68, p<0.05). There was also no significant difference in AHI changes based on dentition status. A negative correlation was found between AHI changes and transverse changes and at canine and molar levels. Conclusion: No significant evidence was found to support the effectiveness of early maxillary expansion in improving sleep apnea among children with maxillary constriction. However, the gender-specific responses and the potential dose-response relationship between maxillary expansion and AHI reduction highlight the complexity of treating pediatric OSA. |

ORIGINAL ARTICLE Impact of orthodontic appliances on patients’ daily lives: a comparison between clear aligners and fixed appliances OLIVEIRA, Priscilla Carvalhal de VILELLA, Beatriz de Souza MARTINS E MARTINS, Mariana MATTOS, Cláudia Trindade de NASCIMENTO, Vanessa de Couto VILELLA, Oswaldo de Vasconcellos Resumo em Português: RESUMO Objetivo: Este estudo teve como objetivo comparar o impacto do tratamento com alinhadores estéticos versus aparelhos fixos convencionais na vida cotidiana de pacientes ortodônticos durante os primeiros seis meses e nos últimos seis meses de tratamento. Métodos: O estudo envolveu 112 pacientes adultos, divididos em quatro grupos. Todos os participantes responderam ao questionário “Dental Impacts on Daily Living” (DIDL), que incluía 36 perguntas e abrangia cinco domínios, incluindo aparência, dor, conforto, desempenho geral e alimentação/mastigação. Estatísticas descritivas e o teste de Mann-Whitney foram utilizados para analisar a pontuação total de cada domínio, enquanto o teste exato de Fisher foi utilizado para comparar os grupos quanto à frequência de respostas “insatisfeito”, “relativamente satisfeito” e “satisfeito”. Resultados: Os resultados não mostraram diferenças significativas (p>0,05) nos escores totais ou nos escores de cada domínio, exceto para o domínio “Desempenho geral/Relacionamento interpessoal”, durante as fases inicial e final do tratamento com aparelhos fixos, indicando que os pacientes se adaptaram ao aparelho ao longo do tempo. A fase inicial do tratamento com aparelhos fixos apresentou o maior percentual de insatisfação. Conclusão: Ambos os tratamentos apresentaram redução no nível de insatisfação relatado pelos pacientes entre os estágios inicial e final, indicando que a adaptação é um fator crucial no impacto dos aparelhos ortodônticos na vida cotidiana dos pacientes. Portanto, os ortodontistas devem estar especialmente atentos às necessidades dos pacientes no período imediatamente posterior à instalação do aparelho.Resumo em Inglês: ABSTRACT Objective: This study aimed to compare the impact of treatment with clear aligners versus conventional fixed appliances on the daily lives of orthodontic patients during the first six months and the last six months of treatment. Methods: The study involved 112 adult patients who were divided into four groups. All participants completed the “Dental Impacts on Daily Living” (DIDL) questionnaire, which included 36 questions and covered five domains including appearance, pain, comfort, general performance and eating/chewing. Descriptive statistics and the Mann-Whitney test were used to analyze the total the scores for each domain, while Fisher’s exact test was used to compare the groups regarding the frequency of “dissatisfied”, “relatively satisfied”, and “satisfied” responses. Results: The results showed no significant differences (p>0.05) in the total scores or in the scores for each domain, except for the “General performance/Interpersonal relationship” domain during the initial and final phases of treatment with fixed appliances, indicating that patients adapted to the appliance over time. The initial phase of treatment with fixed appliances presented the highest percentage of dissatisfaction. Conclusion: Both treatments showed a decrease in the level of dissatisfaction reported by patients between the initial and final stages, indicating that adaptation is a crucial factor in the impact of orthodontic appliances on patients’ daily lives. Therefore, orthodontists must be especially attentive to the needs of patients in the period immediately after the device is installed. |

ORIGINAL ARTICLE Effect of different attachment configurations and skeletal anchorage force application points on canines during distalization of maxillary second molars with clear aligners: a finite element study DOMINGOS, Nathalia de Oliveira SILVA, Douglas Teixeira da KIM, Ki Beom URSI, Weber José da Silva ALMEIDA, Guilherme de Araújo Resumo em Português: RESUMO Objetivo: Este estudo investigou a influência de attachments nos dentes posteriores superiores usando alinhadores e ancoragem esquelética extra alveolar, combinada com precision cuts (PC) ou botões (BT) nos caninos superiores. Métodos: Modelos virtuais 3D foram criados a partir de uma tomografia de um paciente adulto jovem, caucasiano, com dentição permanente intacta desde os segundos molares e apresentando má oclusão de Classe II, divisão 1. Foram utilizados seis modelos de elementos finitos da arcada superior, contendo alinhadores e parafusos extra-alveolares entre os molares superiores, para simular a distalização dos segundos molares com três configurações de fixação (SA, sem attachments; AV, attachments verticais; AVH, attachments verticais e horizontais). PC ou BT foi utilizado para inserir uma força de ancoragem de 1,66 Newtons (N) nos caninos. Foi empregada uma ativação de distalização de 0,2mm entre os molares. Os deslocamentos dentários foram medidos em milímetros (mm) na ponta da cúspide do segundo molar, canino e incisivo central nos eixos sagital (Y), vertical (Z) e coronal (X). Resultados: O segundo molar superior distalizou em todos os modelos e os dentes anteriores perderam ancoragem. Nos modelos sem e com attachments, a relação distalização/perda de ancoragem foi de quase 5:1 e 4:1, respectivamente. Na maioria dos modelos, os dentes avaliados intruíram e moveram-se em direção ao plano sagital. A exceção foi o modelo 3 (PC + AV), no qual os molares foram extruídos. Conclusões: A perda de ancoragem ocorre durante a distalização do segundo molar superior, com ou sem attachments, mesmo na presença de ancoragem esquelética. Sua ocorrência tende a ser menor na ausência de attachments.Resumo em Inglês: ABSTRACT Objective: This study investigated the influence of attachments on the maxillary posterior teeth using aligners and extra alveolar skeletal anchorage applied to precision cuts (PC) or buttons (BT) on the upper canines. Methods: 3D virtual models were created from a tomography of a young adult Class II, division 1 Caucasian patient with healthy and non-restored full complement of permanent teeth (except third molars) was used. Six finite element models of the maxillary teeth with aligners and extra alveolar screw anchorage to simulate second molar distalization with three attachment configurations were used (NA, no attachments; VA, vertical attachments; VHA, vertical and horizontal attachments). PC or BT was used to transfer a anchoring force of 1.66 Newtons (N) on the canines. A 0.2mm of distalization activation was used between molars. Tooth displacements were measured in millimeters (mm) at the cusp tip of the second molar, canine, and central incisor in the coronal (X), sagittal (Y), and vertical (Z) axes. Results: The upper second molar distalized in all models resulting in anchorage loss of anterior teeth. In models without and with attachments, the distalization/anchorage loss ratio was 5:1 and 4:1, respectively. In most models, the teeth evaluated intruded and moved towards the midsagittal plane. The exception was model 3 (PC + VA), in which the molars were extruded. Conclusions: The distalization of upper second molars results in anchorage loss, whether attachments are present or not, despite the presence of skeletal anchorage. Its occurrence tends to be lower in the absence of attachments. |

ORIGINAL ARTICLE Three-dimensional evaluation of facial attractiveness of skeletal Class II patient: comparative analysis after orthodontic and surgical simulations OLIVEIRA, Márcio Bastos de RÊGO, Emanuel Braga SANTOS, Sara Ramos Braga ESTEVES, Lucas Senhorinho CANGUSSÚ, Maria Cristina Teixeira SANTOS, Jean Nunes dos Resumo em Português: RESUMO Objetivo: O objetivo deste estudo foi avaliar a percepção da atratividade facial em pacientes com Classe II esquelética e simular possíveis tratamentos para essa condição. Métodos: Foram utilizados escaneamentos dentários, escaneamentos faciais e tomografia computadorizada de feixe cônico (TCFC) de um paciente do sexo masculino. Essas imagens foram agrupadas em um software, onde o arquivo original foi editado para obter um perfil que apresentasse as características esqueléticas e dentárias de Classe II (1ª divisão) mais pronunciadas, bem como os diferentes tratamentos propostos: Ortodontia compensatória com extração dos primeiros pré-molares superiores; Ortodontia compensatória associada ao avanço cirúrgico do mento e Ortodontia associada à cirurgia ortognática combinada de maxila, mandíbula e rotação anti-horária do plano oclusal. Essas simulações foram apresentadas em forma de vídeos e acompanhadas de um questionário sobre atratividade. Resultados: Os vídeos foram analisados por 275 avaliadores: 109 ortodontistas, 91 cirurgiões bucomaxilofaciais e 165 leigos. Os dados coletados foram submetidos aos testes de Análise de Variância (ANOVA) e t de Student, com nível de significância de 95%. Indivíduos com maior escolaridade são mais rigorosos em sua avaliação estética, mas a face mais atraente para todos os grupos foi alcançada pelo preparo ortodôntico associado à cirurgia ortognática combinada, enquanto a menos atraente foi a Classe II esquelética sem tratamento. Conclusões: Este estudo indica que a face resultante de uma Classe II esquelética é esteticamente desfavorável e os tratamentos geram melhores resultados estéticos, especialmente o preparo ortodôntico associado à cirurgia ortognática combinada.Resumo em Inglês: ABSTRACT Objective: The aim of this study was to assess the perception regarding the attractiveness of the face in patients with skeletal Class II and simulations of possible treatments for this condition. Methods: Dental scans, face scans and cone beam computed tomography (CBCT) of a male patient were used. These images were grouped in a software, where the original file was edited in order to achieve a profile that had the most pronounced skeletal and dental Class II (division 1) features, as well as the different proposed treatments: Compensatory orthodontics with extraction of the maxillary first premolars; Compensatory orthodontics associated with surgical advancement of the chin; and Orthodontics associated with combined orthognathic surgery of the maxilla, mandible and counterclockwise rotation of the occlusal plane. These simulations were presented in the form of videos and accompanied by a questionnaire regarding the attractiveness. Results: The videos were analyzed by 275 evaluators: 109 orthodontists, 91 oral and maxillofacial surgeons and 165 laypersons. The collected data were submitted to analysis of variance (ANOVA) and Student’s t tests, with a significance level of 95%. Individuals with higher levels of education were more rigorous in their aesthetic evaluation, but the most attractive face for all groups was achieved by orthodontic preparation associated with combined orthognathic surgery, while the least attractive was skeletal Class II without treatment. Conclusions: This study indicate that the face resulting from a skeletal Class II is aesthetically unfavorable and the treatments generate better aesthetic results, especially the orthodontic preparation associated with combined orthognathic surgery. |

ORIGINAL ARTICLE Influence of low viscosity resin infiltrant on the adhesion of orthodontic attachments BARRETO, Bruna Caroline Tomé VIDAL, Gabriela Drago SILVER, Katherine Judith de Carvalho Macário Presado LEITE, Karla Lorene de França MONTEIRO, Carolina Mara Geraldino PITHON, Matheus Melo SOUZA, Margareth Maria Gomes de Resumo em Português: RESUMO Introdução: A busca por materiais e técnicas que promovam melhor adesão ao esmalte previamente afetado por lesões cariosas tem se intensificado na Ortodontia, especialmente com o uso de resinas infiltrantes e compósitos com diferentes características. Objetivo: Avaliar a adesão de attachments ortodônticos confeccionados com diferentes compósitos em esmalte previamente infiltrado. Métodos: Noventa incisivos inferiores bovinos foram divididos aleatoriamente em 6 grupos (n=15): 1 controle Z350 (grupo C) e 5 experimentais: Z350 (grupo Z), bulk fill - 3M (grupo FM), bulk fill flow - 3M (grupo FFM), bulk fill - FGM (grupo FF), bulk fill flow - FGM (grupo FFF). Cada grupo diferiu quanto ao tipo de compósito utilizado na confecção dos attachments. Somente os grupos experimentais foram submetidos ao desafio cariogênico seguido de tratamento com Icon, um infiltrante resinoso de baixa viscosidade. Após o envelhecimento, attachments ortodônticos foram colados em todas as amostras (n=90) e realizado o teste mecânico de resistência ao cisalhamento, seguido da análise do índice de remanescente adesivo (IRA). Os dados foram analisados no software Jamovi versão 2.3, com nível de significância de 5%. Foram aplicadas estatísticas descritivas (médias e desvios-padrão), teste de normalidade e ANOVA/Tukey para as variáveis de cisalhamento e Kruskal-Wallis para o IRA. Resultados: O grupo controle apresentou maior força necessária para o cisalhamento dos acessórios (100 N ± 32,6; p = 0,016), enquanto o grupo que utilizou o compósito Bulk Fill Flow FGM apresentou menor força (55,6 N ± 22,5; p = 0,016). Conclusão: A presença do infiltrante resinoso influenciou a adesão dos acessórios ortodônticos confeccionados com diferentes compósitos. Contudo, não interferiu no IRA.Resumo em Inglês: ABSTRACT Introduction: The search for materials and techniques that promote better adhesion to enamel previously affected by carious lesions has intensified in orthodontics, especially with the use of infiltrating resins and composites with different characteristics. Objective: To evaluate the adhesion of orthodontic attachments made with different composites bonded to previously infiltrated enamel. Methods: Ninety bovine lower incisors were randomly divided into 6 groups (n=15): 1 control Z350 (group C) and 5 experimental: Z350 (group Z), bulk fill - 3M (group FM), bulk fill flow - 3M (group FFM), bulk fill - FGM (group FF), bulk fill flow - FGM (FFF). Each group differed by the type of composite used to attachment. Only the experimental groups were subjected to cariogenic challenge followed by treatment with Icon, a low viscosity resinous infiltrant. After aging, orthodontic attachments were bonded to all specimens (n=90) and a mechanical shear bond strength test was performed with subsequent analysis of the adhesive remnant index (ARI). Data were analyzed using Jamovi software version 2.3 with a significance level of 5%. Descriptive statistics (means and standard deviations), normality test and ANOVA/Tukey for shear variables and Kruskal-Wallis test for ARI were performed. Results: The control group required greater force to shear the attachment (100N ± 32.6; p= 0.016), while the group that used the Bulk Fill Flow FGM composite required less force (55.6N ± 22.5; p= 0.016). Conclusion: The presence of resin infiltrant influenced the adhesion of orthodontic attachments, made with different composites. However, it did not interfere with ARI. |

ORIGINAL ARTICLE Dimensional changes of alveolar bone after orthodontic expansion with Invisalign® aligners: study by Cone Beam Computed Tomography VIEIRA, Bruno Boaventura MARAÑÓN-VÁSQUEZ, Guido Artemio FIGUEIREDO, Marcio Antonio de STUANI, Maria Bernadete Sasso ROMANO, Fábio Lourenço MATSUMOTO, Mirian Aiko Nakane Resumo em Português: RESUMO Objetivo: Avaliar as alterações, por meio de Tomografia Computadorizada de Feixe Cônico (TCFC), no osso alveolar vestibular e palatino de pré-molares superiores em indivíduos adultos jovens que foram submetidos à expansão dentoalveolar com alinhadores Invisalign® e compará-las nos diferentes tipos faciais. Método: Quarenta e cinco pacientes (32 mulheres e 13 homens, média de 34,2 anos) realizaram expansão ortodôntica com alinhadores dentários. TCFCs foram realizadas antes (T0) e após expansão do arco superior (T1), período médio de tratamento de 21,4 meses. Foram avaliadas as alterações ósseas alveolares, em altura e espessura, das regiões cervical, média e apical de pré-molares superiores nos diferentes tipos faciais (mesofacial, braquifacial e dolicofacial). O nível de significância adotado foi de 5%. Resultados: Em relação à espessura óssea, houve diferença significante entre os tempos T0 - T1 para os dente 15 e 24 do grupo mesofacial; dente 25 do grupo dolicofacial e dentes 14, 15, 24 e 25 para o grupo braquifacial. Em relação à altura óssea, houve diferença para o dente 15 do grupo braquifacial. Não houve diferença em nenhuma das medidas de altura e espessura óssea comparando-se os diferentes tipos faciais. Conclusão: Verificaram-se pequenas alterações ósseas alveolares, em altura e/ou espessura, de pré-molares superiores para todos os tipos faciais entre os tempos avaliados. Não foram observadas diferenças nas alterações ósseas alveolares entre os tipos faciais; porém, quando analisados separadamente, foram observadas maiores alterações na espessura e altura alveolares no grupo de crescimento horizontal.Resumo em Inglês: ABSTRACT Objective: This study aimed to evaluate changes, using Cone-Beam Computed Tomography (CBCT), in the buccal and palatal alveolar bone of maxillary premolars in young adult individuals who underwent dentoalveolar expansion with Invisalign® aligners, and to compare these changes across different facial types. Methods: Forty-five patients (32 women and 13 men; mean age: 34.2 years) underwent orthodontic expansion with clear aligners. CBCT scans were obtained before (T0) and after expansion of the maxillary arch (T1), with a mean treatment period of 21.4 months. Changes in alveolar bone height and thickness were assessed at the cervical, middle, and apical regions of the maxillary premolars across different facial types (mesofacial, brachyfacial, and dolichofacial). The level of significance was set at 5%. Results: Regarding bone thickness, significant differences between T0 and T1 were observed for tooth 15 and 24 in the mesofacial group; tooth 25 in the dolichofacial group; and teeth 14, 15, 24, and 25 in the brachyfacial group. Regarding bone height, a significant difference was found for tooth 15 in the brachyfacial group. No differences were observed in bone height or thickness when comparing the different facial types. Conclusion: Minor alveolar bone changes in height and/or thickness of maxillary premolars were observed across all facial types between the evaluated time points. No significant differences in alveolar bone alterations were found among the facial types; however, when analyzed separately, greater changes in alveolar thickness and height were observed in the horizontal growth group. |

Apesar de previsível e estável este protocolo requer longo tempo de tratamento, e pode comprometer temporariamente a estética do paciente. A cirurgia de benefício antecipado elimina ou reduz a etapa inicial do tratamento ortodôntico proporcionando uma melhora estética e funcional mais imediata. Objetivo: Apresentar um protocolo de diagnóstico e planejamento virtual, combinando tomografia computadorizada de feixe cônico (TCFC) e modelos digitais aplicado a cirurgias de benefício antecipado com o uso de alinhadores ortodônticos. Método: dez pacientes com deformidade dentofaciais foram submetidos ao protocolo proposto. A acurácia do planejamento virtual foi avaliada pela comparação de 30 medidas cefalométricas planejadas e as obtidas no pós-operatório, nos eixos “x” (transversal), “y” (antero-posterior) e “z” (vertical) utilizando pontos anatômicos de referência. Resultados: A maioria das medidas não apresentou diferenças estatisticamente significativas entre o planejamento e o resultado cirúrgico. As exceções foram o forame incisivo no eixo z (diferença média de -1,79, P=0,045) e o gônio esquerdo no eixo x (diferença média de -1,52, P=0,029). Conclusão: O protocolo de cirurgia de benefício antecipado com alinhadores proposto demonstrou ser uma abordagem viável, segura e eficaz ao tratamento ortodôntico-cirúrgico convencional, com bons índices de previsibilidade clínica.Resumo em Inglês: ABSTRACT Introduction: Traditionally, orthodontic-surgical treatment is performed in three stages: presurgical orthodontics, orthognathic surgery and post-surgical orthodontics. Though predictable and stable, this protocol requires a long treatment time and can temporarily compromise the patient’s aesthetics. Early benefit surgery eliminates or reduces the initial stage of orthodontic treatment, providing more immediate aesthetic and functional improvement. Objective: To present a virtual diagnosis and planning protocol, combining cone beam computed tomography (CBCT) and digital models applied to early benefit surgeries using orthodontic aligners. Method: Ten patients with dentofacial deformities underwent the proposed protocol. The accuracy of virtual planning was assessed by comparing 30 planned cephalometric measurements with those achieved postoperatively, on the “x” (transverse), “y” (anteroposterior) and “z” (vertical) axes, using anatomical reference points. Results: Most measurements did not present statistically significant differences between planning and the surgical outcome. The exceptions were the incisive foramen on the z axis (mean difference = -1.79, P=0.045) and the left gonion on the x axis (mean difference = -1.52, P=0.029). Conclusion: The proposed early benefit surgery protocol with aligners proved to be a viable, safe and effective approach to conventional orthodontic-surgical treatment, with good clinical predictability rates. |
