ORIGINAL ARTICLE Phenotypic characterization of Class III malocclusion using three-dimensional analysis on a sample of Yemeni population: a retrospective cross-sectional study BUKAREZA, Farida AL-HARAZI, Ghamdan AL-RAI, Sarah Resumo em Português: RESUMO Introdução: A má oclusão de Classe III é uma das anomalias no desenvolvimento craniofacial que pode ser afetada principalmente por componentes genéticos e ambientais. Constitui um desafio clínico, devido à compreensão limitada de sua etiologia. Explorar sua diversidade prototípica pode ajudar a identificar seus detalhes etiológicos. Objetivo: O objetivo deste estudo foi caracterizar os subgrupos da má oclusão de Classe III, dependendo das características fenotípicas, usando a tomografia computadorizada de feixe cônico (TCFC) em um grupo selecionado de indivíduos do Iêmen. Métodos: Foi realizado um estudo transversal retrospectivo usando TCFCs pré-tratamento de 80 pacientes (46 homens, 34 mulheres) com idades de ≥18 anos para homens e ≥16 anos para mulheres. Todos os pacientes tinham má oclusão de Classe III, variando de leve a grave. No total, 74 medidas foram analisadas tridimensionalmente com o software Invivo® 6.0. Essas medidas foram categorizadas em 46 variáveis esqueléticas, 18 dentoalveolares e 10 de tecidos moles. A análise de componentes principais (ACP) e a análise de clusters (AC) foram realizadas para identificar os agrupamentos mais comuns nos fenótipos esqueléticos da má oclusão de Classe III. Resultados: A ACP revelou 8 modelos de eixos, que foram responsáveis por 78,9% da variação dos dados produzidos a partir das 74 variáveis. Os quatro primeiros componentes foram responsáveis por 56% das variações totais e explicaram principalmente as relações sagital, dentária, vertical e anteroposterior nos dados. A AC revelou quatro fenótipos esqueléticos de má oclusão de Classe III: C1=32,8%; C2=28,4%; C3=28,4%; e C4=10,4%. Conclusões: Com base nas TCFCs, foram identificados quatro fenótipos de má oclusão esquelética de Classe III na população do Iêmen. Esses achados ajudam a fornecer um diagnóstico diferencial que leva ao estabelecimento de um plano de tratamento mais preciso e eficaz.Resumo em Inglês: ABSTRACT Introduction: Class III malocclusion is one of the abnormalities in the craniofacial development that can be affected mainly by genetic and environmental components. It is a clinical challenge, due to limited understanding of its etiology. Exploring its prototypical diversity helps to identify its etiological details. Objective: This study aimed to characterize Class III malocclusion subgroups depending on the phenotypic characteristics, by using cone beam computed tomography (CBCT) in a selected group of Yemeni subjects. Methods: A retrospective cross-sectional study was performed using 80 pretreatment CBCT of patients (46 males, 34 females), with ages of ≥18 years for males and ≥16 years for females. All cases had Class III malocclusion ranging from mild to severe. The total of 74 measurements were three-dimensionally analyzed using Invivo® 6.0 software. These measurements were categorized into 46 skeletal, 18 dentoalveolar, and 10 soft tissue variables. Principal component analysis (PCA) and cluster analysis (CA) were performed to identify the most common clusters in skeletal Class III malocclusion phenotypes. Results: The PCA revealed 8 axis models, which were responsible for 78.9% of the variation of the data produced from the 74 variables. The first four components accounted for 56% of the total variations, explained mainly the sagittal, dental, vertical, and anteroposterior relationships in the data. The CA revealed four skeletal Class III malocclusion phenotypes: C1=32.8%; C2=28.4%; C3=28.4%; and C4=10.4%. Conclusion: Based on CBCT, four phenotypes of skeletal Class III malocclusion were identified among Yemeni population. These findings help to provide differential diagnosis that lead to set up an accurate and effective treatment plan. |

ORIGINAL ARTICLE Evaluation of reliability and reproducibility of artificial intelligence-based, computer-based and traditional manual tracing methods ARSLAN, Emel SIRIN, Tugce OKTAY, Husamettin KILINC, Delal Dara Resumo em Português: RESUMO Introdução: Os avanços tecnológicos afetam todos os aspectos da vida e da ciência, inclusive os métodos de avaliação cefalométrica em Ortodontia. Aparelhos portáteis baseados em inteligência artificial oferecem aos dentistas a conveniência de analisar seus pacientes a qualquer hora e em qualquer lugar. Objetivo: O objetivo desse estudo foi comparar os métodos de análise cefalométrica de radiografias laterais por Rastreamento com Inteligência Artificial (Artificial Intelligence Tracking Method [AITM], WebCeph), por Rastreamento Baseado em Computador (Computer-Based Tracking Method [CBTM], NemoCeph) e do por Rastreamento Manual (Manual Tracking Method [MTM]), para avaliar a confiabilidade dessas abordagens tecnológicas, em comparação com os métodos tradicionais. Métodos: Duzentas e cinquenta radiografias cefalométricas laterais foram avaliadas usando os três métodos diferentes: WebCeph, NemoCeph e traçado manual. Para isso, foram medidos dez parâmetros angulares (SNA, SNB, ANB, Go-Gn/SN, Plano Oclusal/SN, U1/SN, U1/NA, L1/Plano Mandibular, L1/NB e Ângulo Interincisal [U1/L1]) e cinco lineares (U1/NA, L1/NB, Avaliação de Wits, Ls/Plano E e Li/Plano E), e os dados foram analisados estatisticamente. Resultados: Houve excelente confiabilidade entre os três métodos nas medições para SNA, SNB, ângulo L1.NB, L1/NB mm, ANB, avaliação de Wits, Ls/plano E, Li/plano E, Go-Gn/SN e plano oclusal/SN (p<0,001 e intervalo ICC de 0,915 a 0,979). Com relação à concordância entre as duas medições, houve concordância perfeita e estatisticamente significativa em todos os parâmetros, com exceção de dois moderados. Foram encontradas diferenças estatisticamente significativas entre os métodos de rastreamento em quase todos os parâmetros medidos. Conclusão: Houve diferenças significativas entre os métodos de medição na maioria das medições. Com relação à reprodutibilidade, os valores do ângulo interincisal apresentaram baixa consistência com o método AITM, e os valores de U1/SN apresentaram baixa consistência com os métodos CBTM e AITM.Resumo em Inglês: ABSTRACT Introduction: Technological developments affect every aspect of life and science, including cephalometric evaluation methods in orthodontics. Artificial-intelligence-supported mobile Internet devices provide doctors the convenience of performing necessary analyses for patients anytime and anywhere. Objective: This study aimed to compare the lateral cephalometric measurement methods of the Artificial Intelligence Tracking Method (AITM, WebCeph), Computer-Based Tracking Method (CBTM, NemoCeph), and Manual Tracking Method (MTM), to evaluate the reliability of medical technological approaches, compared to traditional methods. Methods: Two hundred fifty lateral cephalometric radiographs were evaluated using three different methods: WebCeph, NemoCeph, and manual tracing. For this purpose, ten angular (SNA, SNB, ANB, Go-Gn/SN, Occlusal Plane/SN, U1/SN, U1/NA, L1/Mandibular Plane, L1/NB, and Interincisal angle [U1/L1]) and five linear (U1/NA, L1/NB, Wits Appraisal, Ls/E Plane, and Li/E Plane) parameters were measured, and the data were statistically analyzed. Results: There was excellent reliability between the three methods in measurements of SNA, SNB, L1/NB Angle, L1/NB mm, ANB, Wits Appraisal, Ls/E Plane, Li/E Plane, Go-Gn/SN, and Occlusal Plane/SN (p<0.001 and ICC range 0.915 to 0.979). Regarding the agreement between the two measurements, there were statistically significant perfect agreements in all other parameters, except for two moderate ones. Statistically significant differences were found in almost all measured parameters between the tracing methods. Conclusion: There were significant differences in the majority of the measurement methods. Regarding reproducibility, the Interincisal Angle values showed low consistency with the AITM method, and the U1/SN values showed low consistency with both the CBTM and AITM methods. |

ORIGINAL ARTICLE Comparison of facial soft tissue thickness, pharyngeal airway widths in different skeletal Patterns LOUIS, Chraseeta RAGHAVA, Siddharth SHETTY, Praveena MANDOLIL, George S. S-ELANJIKKAL Resumo em Português: RESUMO Métodos: A amostra foi dividida em Padrões esqueléticos I, II e III, com base no ANB e na avaliação de Wits: Padrão I, ângulo ANB = 0-4° e BO à frente do AO em 1 mm nos homens (DP = 1,17 ± 1,9 mm) e 0 mm nas mulheres (DP = -0,10 ± 1,77 mm); Padrão II, ANB >4° e BO localizado atrás do AO (43 indivíduos); e Padrão III, ANB < 0° e BO à frente do AO (43 indivíduos). Cada grupo teve 43 cefalogramas laterais, com um total de 129 cefalogramas. As amostras foram incluídas nos grupos somente se satisfizessem os critérios de ANB e avaliação de Wits. Para isso, 26 pontos de referência selecionados foram identificados e, usando 13 medidas lineares, a espessura dos tecidos moles faciais e a largura das vias aéreas faríngeas foram medidas e comparadas entre os diferentes padrões esqueléticos. Resultados: Houve diferença estatisticamente significativa na espessura dos tecidos moles faciais nos diferentes padrões esqueléticos. O Labrale superius e o Estômio foram significativamente maiores no Padrão III e menores no Padrão II. O Labrale inferius foi significativamente maior no Padrão II e menor no Padrão III. Embora não estatisticamente significativa, a espessura no Mento, Pogônio e Gnátio foi maior no Padrão II, média no Padrão I e menor no Padrão III. Foi observado dimorfismo sexual: os homens apresentaram maior espessura de tecidos moles do que as mulheres, em todos os locais. Houve diferença estatisticamente significativa nas larguras das vias aéreas faríngeas nos diferentes padrões esqueléticos. As larguras das vias aéreas superiores e inferiores da faringe foram significativamente maiores no Padrão III e menores no Padrão II. Conclusão: Ao desenvolver planos de tratamento ortodôntico, as diferenças dos tecidos moles e as larguras das vias aéreas faríngeas precisam ser levadas em conta como fatores cruciais. Os resultados do presente estudo indicam que houve uma correlação substancial entre a espessura dos tecidos moles da face, o diâmetro da faringe e os diferentes padrões esqueléticos.Resumo em Inglês: ABSTRACT Methods: The sample was divided into skeletal Patterns I, II and III, based on ANB and Wits appraisal: Pattern I, when ANB angle = 0-4° and BO was ahead AO by 1mm in males (SD = 1.17 ± 1.9mm), 0mm in females (SD = -0.10 ± 1.77mm); Pattern II, when ANB >4° and BO was located behind AO (43 subjects); and Pattern III, ANB < 0° and BO was ahead of AO (43 subjects). Each group had 43 lateral cephalograms, with a total of 129 cephalograms. The samples were divided into three groups only if they satisfied the criteria of both ANB and Wits appraisal. For this, 26 selected landmarks were identified and, using 13 linear measurements, facial soft tissue thickness and pharyngeal airway widths were measured and compared among the different skeletal patterns. Results: There was statistically significant difference in facial soft tissue thickness in the different skeletal patterns. Labrale superius and stomion were statistically significant and higher in Pattern III and smaller in Pattern II. Labrale inferius was found to be statistically significant and higher in Pattern II and smaller in Pattern III. Though not statistically significant, the thickness at Menton, Pogonion, and Gnathion was higher in Pattern II, average in Pattern I and smaller in Pattern III. Sexual dimorphism was noted: Men had a greater soft tissue thickness than women in all sites. There was statistically significant difference in pharyngeal airway widths in different skeletal malocclusions. Both upper and lower pharyngeal airway widths were found to be statistically significant and higher in Pattern III and smaller in skeletal Pattern II. Conclusion: When developing orthodontic treatment plans, soft tissue differences and pharyngeal airway widths need to be taken into account as crucial factors. The findings of the current study indicate that there was a substantial correlation between the thickness of the soft tissues in the face, the diameter of the pharynx, and several skeletal patterns. |

ORIGINAL ARTICLE Gingival changes after removal of fixed orthodontic appliances: a prospective cohort clinical study TAVARES, Luciana Cláudia Diniz GOUVÊA FILHO, Luis Carlos CHAOUBAH, Alfredo MAXIMIANO, Gustavo Silva CAMPOS, Jocimara Domiciano Fartes de Almeida MOTA JÚNIOR, Sergio Luiz VITRAL, Robert Willer Farinazzo CAMPOS, Marcio José da Silva Resumo em Português: RESUMO Introdução: A presença de braquetes ortodônticos pode comprometer a eficiência da higiene bucal, levando a alterações na microbiota bucal, aumentando o risco de alterações gengivais após a remoção do aparelho ortodôntico fixo. Objetivo: O objetivo deste estudo foi avaliar as alterações no volume gengival nos dentes anteroinferiores após a remoção do aparelho ortodôntico fixo. Métodos: A amostra foi composta por 84 dentes anteroinferiores de 14 indivíduos adultos em fase final de tratamento ortodôntico fixo e que tiveram a condição gengival de seus incisivos inferiores registrada por meio de fotografias. Os tempos de avaliação foram de T0 a T4. Em T0, os indivíduos foram submetidos à remoção do cálculo dentário e profilaxia com jato de bicarbonato de sódio, instalação de contenção fixa, radiografia periapical digital dos dentes anteroinferiores e fotografia frontal extrabucal para registro da condição gengival. Em T1, o aparelho foi removido e o biotipo gengival foi determinado. Outros exames fotográficos foram realizados nos tempos T2, T3 e T4. Resultados: Os valores clínicos da coroa dos incisivos apresentaram aumento significativo 21 dias após a remoção do aparelho ortodôntico. A altura da papila entre os incisivos apresentou redução significativa no 14º dia após a remoção do aparelho, enquanto a papila entre o canino e o incisivo lateral apresentou significância apenas na última avaliação. Conclusões: A amostra apresentou predomínio de biotipo gengival fino e as variáveis radiográficas não apresentaram correlação significativa. Houve alterações gengivais na região anteroinferior, com redução espontânea e progressiva de todas as papilas analisadas.Resumo em Inglês: ABSTRACT Introduction: The presence of orthodontic brackets can compromise the efficiency of oral hygiene, leading to changes in oral microbiota, increasing the risk of gingival changes after removal of the fixed orthodontic appliance. Objective: The objective of this study was to evaluate changes in gingival volume in the lower anterior teeth after removal of the fixed orthodontic appliance. Methods: The sample consisted of 84 lower anterior teeth of 14 adult individuals in the final phase of fixed orthodontic treatment and had the gingival condition of their lower incisors recorded through photographs. The evaluation times were from T0 to T4. At T0, the individuals underwent removal of dental calculus and prophylaxis with a sodium bicarbonate jet, installation of a fixed retainer, digital periapical radiography of the anteroinferior teeth and extraoral frontal photograph to record the gingival condition. At T1, the appliance was removed and the gingival biotype was determined. Other photographic exams were performed at times T2, T3 and T4. Results: Clinical incisor crown values showed a significant increase 21 days after orthodontic appliance removal. The height of the papilla between the incisors showed a significant reduction on the 14th day after removal of the appliance, while the papilla between the canine and the lateral incisor showed significance only in the last evaluation. Conclusions: The sample showed a predominance of thin gingival biotype, the radiographic variables showed no significant correlation. There were gingival changes in the anteroinferior region, with spontaneous and progressive reduction of all analyzed papillae. |

ORIGINAL ARTICLE Long-term evaluation (10 years) of the outcomes of Herbst functional appliance in pharyngeal dimensions and hyoid bone position ALMEIDA, Thagid Yasmin Leal FIALHO, Tiago FREITAS, Karina Maria Salvatore de HENRIQUES, José Fernando Castanha FREITAS, Marcos Roberto de Resumo em Português: RESUMO Objetivo: Este estudo teve como objetivo avaliar as alterações a longo prazo nas dimensões faríngeas e na posição do osso hioide em pacientes com má oclusão de Classe II tratados com aparelho funcional Herbst. Material e Métodos: A amostra foi composta por 15 pacientes com má oclusão esquelética de Classe II (13,00 anos ± 1,21) tratados com aparelho funcional Herbst e acompanhados por um período médio de 10 anos (10,73 anos ± 3,67). Radiografias laterais da cabeça foram utilizadas para avaliar as dimensões faríngeas e a posição do osso hioide, e as medidas foram realizadas com o programa Dolphin Imaging 11.9. A comparação intragrupos entre os estágios de avaliação foi realizada com o teste ANOVA de medidas repetidas, seguida do teste de Tukey, se necessário. Os resultados foram considerados estatisticamente significativos em p < 0,05. Resultados: Em relação às variáveis cefalométricas faríngeas, apenas o espaço hipofaríngeo aumentou significativamente no período de tratamento, e todos eles permaneceram estáveis durante o período de acompanhamento. O osso hioide moveu-se significativamente para frente e para baixo durante o período de tratamento e ainda mais para frente durante o período de pós-tratamento de longo prazo. Conclusão: Os resultados do aparelho funcional Herbst nas dimensões faríngeas e na posição do osso hioide são estáveis ao longo dos anos.Resumo em Inglês: ABSTRACT Objective: This study aimed to evaluate the long-term changes in the pharyngeal dimensions and hyoid bone position in Class II malocclusion patients treated with Herbst functional appliance. Material and Methods: The sample comprised 15 skeletal Class II malocclusion patients (13.00 ± 1.21 years) treated with Herbst functional appliance and followed for a mean period of 10 years (10.73 ± 3.67 years). Lateral headfilms were used to evaluate the pharyngeal dimensions and hyoid bone position, and the measurements were performed with Dolphin Imaging 11.9. Intragroup comparison between the evaluation stages was performed with repeated measures ANOVA, followed by Tukey tests if necessary. Results were considered statistically significant at p<0.05. Results: Regarding the pharyngeal cephalometric variables, only the hipo pharynx space increased significantly in the treatment period, and all of them remained stable remained stable during follow-up period. The hyoid bone moved significantly forward and downward during the treatment period and even further forward during the long-term posttreatment period. Conclusion: The outcomes of Herbst functional appliance in pharyngeal dimensions and hyoid bone position are stable over the years. |

ORIGINAL ARTICLE Effect of 35% hydrogen peroxide in promoting enamel whitening with orthodontic attachments bonded on its surface BARRETO, Bruna Caroline Tomé VILELA, Luiza Trindade MARAÑÓN-VÁSQUEZ, Guido Artemio PITHON, Matheus Melo SOUZA, Margareth Maria Gomes de Resumo em Português: RESUMO Introdução: A exigência por estética e o desejo por dentes brancos durante o tratamento ortodôntico é um desejo dos pacientes. Objetivo: Avaliar o efeito do peróxido de hidrogênio a 35% na promoção do clareamento do esmalte com attachments ortodônticos colados em sua superfície. Métodos: 80 incisivos bovinos divididos aleatoriamente em 4 grupos (n=20): (GI) grupo controle, (GII) grupo que recebeu apenas clareamento dental, (GIII) grupo que recebeu apenas colagem de attachments na superfície e (GIV) grupo que recebeu colagem de attachments e clareamento. O clareamento foi realizado seguindo as instruções do fabricante e os attachments eram colados à superfície dentária por meio de um template após condicionamento ácido do esmalte e aplicação de sistema adesivo. A avaliação da cor foi realizada de acordo com a escala de cores LAB da Commission Internationale de l’Eclairage. A análise estatística foi realizada no programa Jamovi adotando nível de significância de 5%. Foi realizada análise descritiva dos dados, caracterizando todas as variáveis do estudo. Modelos lineares gerais ajustados por regressão de mínimos quadrados ordinários foram utilizados para testar o efeito do clareamento, do uso de attachments e da interação de ambos os fatores. Resultados: As variáveis “clareamento” e “uso de attachments” afetam todos os parâmetros da escala CIELAB (mantendo a outra variável constante). O efeito clareador foi independente da presença de attachments. Conclusão: o peróxido de hidrogênio a 35% é capaz de promover o clareamento do esmalte com attachments ortodônticos colados em sua superfície. Relevância Clínica: Apesar de ser uma pesquisa in vitro, o estudo proporciona aos pacientes e profissionais a possibilidade de clareamento dental durante o tratamento ortodôntico com alinhadores.Resumo em Inglês: ABSTRACT Introduction: The demand for aesthetics is high and the desire for white teeth during orthodontic treatment is a desire of patients. Objective: To evaluate the effect of 35% hydrogen peroxide in promoting enamel whitening with orthodontic attachments bonded to its surface. Methods: 80 bovine incisors randomly divided into 4 groups (n=20): (GI) control group, (GII) group that received only tooth whitening, (GIII) group that received only bonding of attachments on the surface and (GIV) group that received bonding of attachments and whitening. Whitening was carried out following the manufacturer’s instructions while the attachments were bonded to the dental surface using a template after acid conditioning of the enamel and application of an adhesive system. Color evaluation was performed according to the LAB color scale of the Commission Internationale de l’Eclairage. Statistical analysis was performed using Jamovi program adopting a 5% significance level. Descriptive data analysis was performed, characterizing all study variables. General linear models adjusted by ordinary least squares regression were used to test the effect of whitening, enhancement use, and the interaction of both factors. Results: Both “whitening” and the use of “attachments” have an effect on all parameters of the CIELAB scale (keeping the other variable constant). The whitening effect was independent of the presence of attachments. Conclusion: 35% hydrogen peroxide is capable of promoting enamel whitening with orthodontic attachments bonded to its surface. Clinical Meaning: Despite being in vitro research, the study provides patients and professionals with the possibility of teeth whitening during orthodontic treatment with aligners. |

ORIGINAL ARTICLE A pilot study comparing virtual treatment setups among clear aligner companies TELLES, Vicente MATTOS, Claudia Trindade COPELLO, Flavio DOUGHAN, Maria B. Resumo em Português: RESUMO Objetivo: Este estudo piloto tem como objetivo comparar o setup virtual entre quatro empresas de alinhadores para avaliar como as características de cada uma podem impactar no plano de tratamento. Métodos: A documentação inicial de 10 pacientes foi enviada para Invisalign®, ClearCorrect®, 3M™ Clarity™, e Spark™. A prescrição de tratamento para cada caso foi padronizada para garantir planos de tratamento equiparáveis entre as empresas. As comparações focaram no número de alinhadores, número de attachments, redução interproximal por arco, movimento vertical dos incisivos centrais superiores, relações oclusais finais de caninos e molares, distâncias intercaninos e intermolar, e expansão ou constrição dessas distâncias. Resultados: Entre as empresas, diferenças significativas foram encontradas no número de alinhadores (p-valor = 0,003), número de attachments (p-valor < 0,001) e relação oclusal final dos caninos (p-valor = 0,013). Não foram observadas diferenças estatísticas para as outras variáveis. A ClearCorrect® prescreveu o menor número de alinhadores e attachments, enquanto a 3M™ Clarity™ prescreveu o maior. A ClearCorrect® e a Spark™ apresentaram deficiências no planejamento para obtenção de relação bilateral de Classe I dos caninos. Conclusões: As características individuais de cada empresa de alinhadores resultam em abordagens distintas para o tratamento de um mesmo paciente, destacando-se tanto áreas de discrepância quanto de consistência nas configurações do setup virtual.Resumo em Inglês: ABSTRACT Objective: This pilot study aims to compare virtual treatment setups provided by four different clear aligner companies, to assess how variations in features might impact treatment planning. Methods: Initial records of 10 patients were submitted to Invisalign®, ClearCorrect®, 3M™ Clarity™, and Spark™. Standardized case prescriptions ensured comparable treatment plans across companies. Comparisons focused on the number of aligners, number of attachments, interproximal reduction per arch, vertical movement of maxillary central incisors, final canine and molar relationships, intercanine and intermolar widths, and expansion or constriction of these widths. Results: Among the companies, significant differences were found in the number of aligners (p = 0.003), number of attachments (p< 0.001), and final canine relationship (p = 0.013). No statistical differences were observed for the other variables. ClearCorrect® prescribed the fewest number of aligners and attachments, while 3M™ Clarity™ prescribed the most. ClearCorrect® and Spark™ showed deficiencies in planning for bilateral canine Class I relationship. ORIGINAL ARTICLE Evaluation of dental enamel after debonding of sapphire brackets and removal of orthodontic adhesive remnants SILVA, Elizabety do Nascimento QUINTANS, Daniel Magalhães DIAS, Rebeca Tibau Aguiar SOUZA, Frederico Barbosa de OLIVEIRA, Rudyard dos Santos Resumo em Português: RESUMO Introdução: Os efeitos iatrogênicos no esmalte dentário são comuns durante a descolagem de braquetes estéticos e a remoção de adesivo residual. Objetivo: Avaliar o impacto desses procedimentos na integridade do esmalte por meio de técnicas avançadas de imagem. Métodos: Coroas de pré-molares humanos (n=60) foram coladas com braquetes de safira e descoladas com os alicates 346R (G1, n=30) e 346B (G2, n=30). A remoção do adesivo foi realizada com uma broca de zircônia multilaminada (GE1, n=30) e uma broca de Carbide multilaminada (GE2, n=30). A rugosidade superficial foi avaliada por meio de perfilometria óptica, perda de esmalte e microfissuras por meio de microtomografia computadorizada e o índice de remanescente adesivo (IRA) por meio de tomografia de coerência óptica. Após os testes de normalidade e homogeneidade, foram aplicados os testes de Mann-Whitney e Wilcoxon (p<0,05). Resultados: O alicate 346R apresentou IRA ligeiramente maior do que o 346B. Ambos os métodos de remoção aumentaram significativamente a rugosidade da superfície sem causar microfissuras no esmalte. A broca de zircônia demonstrou maior eficiência de remoção de adesivo, mas foi associada ao aumento da perda de esmalte. Conclusões: Variações na forma e no vetor de força do alicate não afetaram os resultados do esmalte. O sistema de broca de zircônia em baixa rotação foi mais eficiente na remoção de remanescentes adesivos ortodônticos, resultando em menor IRA final na superfície do esmalte, e nenhum dos procedimentos causou microfissuras no esmalte, conforme avaliado pela microtomografia computadorizada.Resumo em Inglês: ABSTRACT Introduction: Iatrogenic effects on dental enamel are common during the debonding of aesthetic brackets and the removal of residual adhesive. Objective: To evaluate the impact of these procedures on enamel integrity via advanced imaging techniques. Methods: Human premolar crowns (n=60) were bonded with sapphire brackets and debonded using 346R pliers (G1, n=30) and 346B pliers (G2, n=30). Adhesive removal was performed with a multilaminated zirconia bur (EG1, n=30) and a multilaminated carbide bur (EG2, n=30). Surface roughness was assessed via optical profilometry; enamel loss and microcracks, via microcomputed tomography; and the adhesive remnant index (ARI), via optical coherence tomography. After normality and homogeneity tests, Mann-Whitney and Wilcoxon tests were applied (p<0.05). Results: The 346R plier presented a slightly higher ARI than the 346B plier. Both removal methods significantly increased surface roughness, without causing enamel microcracks. The zirconia bur demonstrated greater adhesive removal efficiency but was associated with increased enamel loss. Conclusions: Variations in the shape and force vector of the pliers did not affect enamel outcomes. The zirconia bur system at low speed was more efficient at removing orthodontic adhesive remnants, resulting in a lower final ARI on the enamel surface, and none of the procedures caused enamel microcracks, as evaluated by microcomputed tomography. |

Essa condição afeta cerca de 2% da população e a maioria das ocorrências (entre 80% e 90% dos casos) resulta na impacção no palato. As sequelas de um canino impactado podem incluir migração e reabsorção radicular nos dentes adjacentes, redução do perímetro da arcada, formação de cistos e infecções. Objetivo: Com o propósito de auxiliar a abordagem mais adequada para casos de caninos superiores permanentes não irrompidos e palatinizados, em pacientes jovens, sem a presença de apinhamento ou com apinhamento leve na arcada superior, desenvolveu-se um tutorial interativo, em formato de apresentação do PowerPoint ®. Conclusão: Esse tutorial deve ser utilizado como ferramenta auxiliar, e o tratamento para essa condição dentária deve ser individualizado para as características de cada paciente.Resumo em Inglês: ABSTRACT Introduction: The second most common form of dental impaction is the maxillary permanent canine, behind only the third molars. This condition affects approximately 2% of the population, with most cases (between 80% and 90%) resulting in impaction in the palate. The consequences of a canine impaction may include migration and root resorption in adjacent teeth, reduction of the dental arch perimeter, formation of cysts, and infections. Objective: We developed an interactive PowerPoint tutorial to assist professionals in selecting the most appropriate approach for cases of unerupted and palatally displaced permanent maxillary canines in young patients without or with minimal crowding in the maxillary arch. Conclusion: This tutorial is an auxiliary tool, and the treatment plan for this dental condition should be individualized to the characteristics of each patient. |
