A toxoplasmose é infecção freqüente em todo o mundo, mas na maioria dos casos não traz repercussões importantes para o paciente, exceto indivíduos imunodeprimidos e fetos, os quais podem apresentar graves seqüelas. O diagnóstico precoce durante a gravidez é altamente desejável, já que o tratamento da gestante reduz a freqüência e gravidade da infecção fetal. Neste estudo aplicou-se a técnica de PCR em 86 amostras de líquido amniótico de gestantes que apresentaram soroconversão durante a gravidez. O DNA foi amplificado usando-se iniciadores externos e internos, num sistema de nested PCR. As amostras foram também inoculadas em camundongos e os recém-nascidos acompanhados clinicamente através de sorologia, RX de crânio, ultrassom transfontanela, exame de fundo de olho e punção lombar. Pela PCR, sete casos foram positivos e 79 negativos. Entre os positivos, dois não se confirmaram pela inoculação em camundongo nem pela avaliação clínica da criança; dos negativos, três apresentaram clínica de toxoplasmose congênita, e em um deles o exame de inoculação em camundongo foi positivo. A sensibilidade e especificidade da PCR foram 62,5% e 97,4%, respectivamente; a inoculação em camundongos mostrou 42,9% de sensibilidade e 100% de especificidade. Embora a PCR não deva ser usada como único teste diagnóstico da toxoplasmose congênita, trata-se de método promissor e merece mais estudos para aumentar sua eficácia.

Abstract in English:

Toxoplasmosis is one of the most common infections all over the world. Most cases are asymptomatic, except in immunosuppressed individuals and fetuses, which can be seriously damaged. Prenatal diagnosis should be made as soon as possible since treatment of the mother can minimize fetal sequelae. Our aim in this study was to test the polymerase chain reaction technique (PCR) in 86 samples of amniotic fluid from women who seroconverted during pregnancy. DNA was amplified using external primers and, in a second step, internal primers, in a nested PCR system. Samples were also inoculated into mice and the newborn were evaluated by T. gondii serology, skull x-ray, transfontanel ultrasound, fundoscopic examination, lumbar puncture and clinical examination. PCR was positive in seven cases and negative in 79. Among PCR-positive cases, two were negative by inoculation into mice and by clinical evaluation; among PCR-negative ones, three had clinical evidence of toxoplasmosis and one was positive after inoculation into mice. PCR showed values of sensitivity = 62.5% and specificity = 97.4%; the values of inoculation into mice where 42.9% and 100%, respectively. Although PCR should not be used alone for prenatal diagnosis of congenital toxoplasmosis, it is a promising method and deserves more studies to improve its efficacy.

Para avaliar se os níveis séricos de IFN-gama e TNF-alfa podem ser usados como marcadores da resposta terapêutica na leishmaniose cutânea, 54 pacientes com história de uma única lesão ulcerada com duração de até 30 dias foram arrolados para o estudo. IFN-gama e TNF-alfa foram mensurados por ELISA no sobrenadante de cultura de linfócitos, antes e 60 dias após início da terapia. Cicatrização completa da lesão 60 dias após o tratamento foi considerada critério de cura. Os níveis de IFN-gama e TNF-alfa foram similares em ambos os grupos antes da terapia. Houve tendência de aumento dos níveis de IFN-gama nos pacientes curados aos 60 dias, os valores não alcançam significância estatística. Em ambos os grupos de pacientes, os níveis iniciais de TNF-alfa foram semelhantes antes da terapia e caíram significantemente após tratamento independente da cura ou manutenção da lesão ativa.

Abstract in English:

To evaluate if IFN-gamma and TNF-alpha levels could be used as markers of therapeutic response in cutaneous leishmaniasis, 54 patients with history of one ulcerated cutaneous lesion, with up to 30 days onset, were enrolled in the study. IFN-gammaand TNF-alpha were measured by ELISA in lymphocyte cultures supernatant before and 60 days after initiating therapy. Cure was considered to be a complete healing of lesion 60 days after treatment. IFN-gamma and TNF-alpha levels were similar in both groups of patients before therapy. There was a tendency to increase IFN-gamma levels in patients that were cured in 60 days, however the values did not reach statistical significance. In both groups of patients, TNF-alpha levels were similar before therapy and fell significantly after treatment, irrespective of cure or maintenance of active lesion.
The effects of immunization with recombinant Sm14 (rSm14) in reducing worm burden and mortality of mice infected with Schistosoma mansoni Ribeiro, Fábio Vieira, Cristiane dos Santos Fernandes, Adriana Araújo, Neusa Katz, Naftale

A fim de investigar se a imunização de camundongos com a proteína recombinante de14,7 KDa (Sm14r) de Schistosoma mansoni confere proteção contra uma infecção letal por S. mansoni, camundongos imunizados com Sm14r foram infectados com diferentes cargas de vermes.Uma proteção significativa foi demonstrada nos camundongos imunizados e infectados com 100 ou 1.000 cercárias de S. mansoni quando comparados com os controles (p< 0,004 e p< 0,01 respectivamente). Diferentemente de resultados anteriores, nenhum camundongo do grupo controle (não imunizado e infectado com 1.000 cercárias) morreu antes do 30º dia após infecção. Uma correlação direta entre o número de cercárias e o tempo de morte dos camundongos foi detectada. Anticorpos IgM anti-Sm14r foram produzidos significativamente (p< 0,05), principalmente, nos grupos de camundongos imunizados e infectados com 500 ou 1.000 cercárias. Anticorpos IgG e IgA anti-Sm14r não foram produzidos em quantidades significativas. Os testes de immunoblots demonstraram que todos os soros de camundongos revelaram uma banda específica de aproximadamente 14,7 KDa, e com maior intensidade nos soros dos camundongos imunizados. A maior reatividade foi encontrada no soro no período da terceira imunização.Os resultados mostram que imunização com Sm14r reduz a carga de vermes independentemente da carga de cercárias da infecção. Nenhuma correlação foi encontrada entre tipos de anticorpos e redução da carga de vermes. Uma correlação direta foi encontrada em relação a carga de cercárias e a taxa e precocidade da mortalidade dos camundongos.

Abstract in English:

To investigate whether mice immunization with the recombinant form of a 14.7 KDa Schistosoma mansoni protein (rSm14) confers protection against a S. mansoni lethal challenge infection, rSm14-immunized mice were challenged with different cercarial burdens. A significant protection was detected in immunized mice challenged with 100 or 1,000 S. mansoni cercariae when compared with their controls (p< 0.004 and p< 0.01 respectively). Differently from previous report, none of the mice from the control group (not immunized and infected with 1000 cercariae) died before the 30th day post-infection. A direct correlation between the number of challenge cercariae and the precocity of mice death was found. IgM anti-rSm14 antibodies were significantly produced (p< 0.05) mainly in the groups of immunized mice infected with 500 or 1000 cercariae. IgG and IgA anti-rSm14 antibodies were not significantly detected. In Western immunoblots, all mice sera showed a specific antibody response with a 14.7 KDa antigen being reacted with particular intensity in sera from immunized mice. The results show that immunization with rSm14 reduced mice worm burden independently of the cercariae load of challenge infection. No correlation was found between serum antibodies and worm burden reduction. In relation to cercarial load and the rate and precocity of mice mortality a direct correlation was found.

Os dermatófitos são um grupo de fungos taxonomicamente relacionados que têm a capacidade de invadir os tecidos queratinizados (pele, pêlo e unha) dos homens e animais produzindo infecções denominadas dermatofitoses. Com o intuito de avaliar a epidemiologia e etiologia das infecções causadas por estes fungos em Goiânia, GO, foram examinadas no Laboratório de Micologia do Instituto de Patologia Tropical e Saúde Pública da Universidade Federal de Goiás, de janeiro a dezembro de 1999, 1.955 amostras de indivíduos com suspeita clínica de dermatofitoses. Foram isolados 445 (22,8%) cepas de dermatófitos e identificados principalmente Trichophyton rubrum (49,4%), Trichophyton mentagrophytes (30,8%) e Microsporum canis (12,6%). Quanto à localização das lesões, os membros inferiores, unhas dos pés e couro cabeludo foram as regiões mais acometidas. Neste estudo foram avaliados dados correlacionados a sexo, faixa etária, local das lesões e agente etiológico.

Dermatophytes are a group of closely related fungi that have the capacity to invade keratinized tissue (skin, hair, and nails) of humans and animals to produce infections called dermatophytosis. In order to review the etiology and epidemiology of dermatophytosis in Goiânia, GO, 1955 specimens with diagnostic suspicion of dermatophytic lesions, were collected from January to December, 1999, from the Mycology Laboratory in the Institute of Tropical Pathology and Public Health, Federal University of Goiás. A total of 445 (22.8%) samples were positive for dermatophytes and Trichophyton rubrum was the most frequently isolated species (49.4%) followed by Trichophyton mentagrophytes (30.8%) and Microsporum canis (12.6%). Concerning the location of the lesions, the inferior limbs, feet and nails together with tinea capitis were the most frequently found clinical pattern in the majority of patients. Correlation between sex, age, location of the lesions and etiologic agents is considered in the study.

Abstract in Portuguese:

O fenômeno El Niño, que afetou o Perú em 1998, permitiu o crescimento de abundante vegetação em ambientes tradicionalmente secos. Paederus irritans, coleóptero que produz dermatite ao ser esfregado na pele, encontrou aí um substrato muito favorável ao incremento de sua população. A chegada das chuvas nos meses de fevereiro a abril de 1999 interferiu no hábitat desse inseto, que migrou inclusive para áreas urbanas, condicionando maior exposição da população humana de Piura, no norte do país, ao contato com esse agente. Entre fevereiro e maio de 1999 foram notificados em Piura 1.451 casos da dermatite por Paederus irritans, aí denominada latigazo dado o aspecto característico das lesões, eritematosas, lineares, semelhantes a chicotadas, por ele produzidas na pele por ação da pederina. As áreas do corpo mais afetadas foram a cabeça (56,6%) e o pescoço (30,9%), mas houve casos de conjuntivite e até de lesões genitais devidas à contaminação pelas mãos.

Abstract in English:

The phenomen El Niño that affected Peru at 1998, made possible the growth of copious vegetation in traditionally dry places. On this way, Paederus irritans, dipterous of the order Coleoptera that produces dermatitis when croushed on the skin, found substract that allowed the natural growth of its population. The coming of occasional rains on the months of February to April 1999, attacked the habitat of this insect, that looked for urban areas, exposing seriously the general population of Piura on north Perú to the contact. During February and May 1,451 cases of dermatitis caused by Paederus irritans, called latigazo because of its linear and eritematous characteristic lesions on the skin, were notified at Piura. The more affected body areas were head (56.6%) and neck (30.9%), but there were also cases of conjunctivitis and genital injuries, because of hands contamination.

Estudou-se a correlação entre a positividade do xenodiagnóstico artificial e a quantidade utilizada de sangue e de triatomíneos, em 200 pacientes na fase crônica da doença de Chagas. Colheram-se 10 ou 40ml de sangue através de tubos a vácuo, heparinizados com 20,4UI, sendo realizado com 60 e 360 triatomíneos, respectivamente. Usou-se Dipetalogaster maximus, Triatoma infestans, Triatoma vitticeps e Rhodnius neglectus, nos estádios 1°, 3°, 3° e 4°, respectivamente. A coproscopia dos triatomíneos foi realizada aos 30 e 60 dias após a aplicação. A positividade do xenodiagnóstico com o primeiro e o segundo métodos foi de 19% e 44%, respectivamente, sendo altamente significativa a correlação entre a quantidade utilizada de sangue e triatomíneos e a positividade (p<0,01). A xenopositividade nas faixas etárias variou de 9,7 a 100%, sendo maior em jovens e adultos até 34 anos, e independente em relação ao sexo dos pacientes.

Abstract in English:

The aim of this study was to verify whether the amount of blood and number of triotomines used could improve the artificial xenodiagnosis performed in 200 chronic phase infected individuals. Ten or 40ml of peripheral blood was collected in heparinized (20.4 IU) vacuum tubes, and fed to 60 and 360 triatomines, respectively. Dipetalogaster maximus (1st instar, about 15 days after eclosion), as well as 3rd instar Triatoma infestans and Triatoma vitticeps and the 4th instar of Rhodnius neglectus were used. The faecal examinations were performed 30 and 60 days after xenodiagnosis procedure. The positivity with 10ml of blood was 19% and with 40ml, 44%, from which it was concluded that a correlation existed between the amount of blood and the number of triatomines used (p < 0.01). The positivity of the xenodiagnosis ranged from 9.7 to 100%, higher in young and adults up to 34 years old, but independent in relation to the sex of the chronic chagasic individuals studied.

Schistosoma mansoni: the action of lovastatin on the murine model Araújo, Neusa Kohn, Anna Oliveira, Áureo Almeida de Katz, Naftale

Com o objetivo de avaliar a ação da lovastatina na postura do Schistosoma mansoni foram usados camundongos infectados com 100 ±10 cercárias da cepa LE. Trinta dias após a infecção, os animais foram tratados com 100, 200 e 400mg/kg de lovastatina, via oral, durante cinco dias consecutivos e sacrificados 7,15,30 ou 60 dias após o tratamento. Foram analisados: distribuição de vermes no mesentério e fígado, mortalidade de vermes no fígado, alteração do oograma, contagem de ovos no jejuno e fígado, presença de ovos intra-uterino e morfologia dos vermes dos grupos tratados e controle (animais infectados e não tratados). Diferenças estatisticamente significativas foram encontradas, na dose de 400mg/kg, entre grupos tratados e controles quando se considerou a presença de ovos no útero, a alteração do oograma, ovos nos diferentes estágios de desenvolvimento no jejuno e fígado e no comprimento do corpo dos vermes machos e fêmeas. O estudo morfológico dos vermes mostrou alterações degenerativas sendo as principais no aparelho reprodutor, com redução e alteração dos folículos vitelínicos e do ovário das fêmeas e modificações nos testículos dos machos. Os resultados apresentados levam à conclusão que a droga em estudo reduz, consideravelmente, a oviposição das fêmeas do S. mansoni, aumenta o tamanho dos vermes, provoca alterações no sistema reprodutivo de machos e fêmeas e pode ocasionar morte de parte significativa da população de vermes na dose de 400mg/kg.

Abstract in English:

Aimed to evaluate the lovastatin action on Schistosoma mansoni oviposition, infected mice with 100 ± 10 cercariae of the LE strain were used. Thirty days after infection the animals were treated with 100, 200 and 400mg/kg of lovastatin, per os, during five consecutive days and then sacrificed 7, 15, 30 or 60 days after treatment. We analyzed: distribution of worms in mesenteries and liver; mortality of worms in the liver; alteration of the oogram; eggs counting in the jejunum and liver; presence of intrauterine eggs and morphology of the worms from the treated and control groups (infected and not treated animals). Significant statistical differences were found between treated and control groups when the presence of intrauterine eggs was considered and also, alteration on the oogram, eggs at different stages of development in jejunum, liver and in the body length of males and females. The morphological study of the worms showed that the degenerative modifications occurred, mainly in the reproductive system, with reduction and alteration of the viteline follicles and the ovary of the females. Also modifications in the males testicles were observed. The results suggest that the drug under study reduces, considerably, the female S. mansoni, oviposition increases the worms' size, leads to alteration in the reproductive system of males and females. It also may induce death of a significant part of the worm populations at the dose of 400mg/kg.

Abstract in Portuguese:

Pacientes portadores de megaesôfago chagásico foram submetidos à endoscopia digestiva alta e coleta do material com pinças e sondas especiais, em condições estéreis. Foram realizadas quatro coletas, sendo uma do líquido de estase e três fragmentos da mucosa esofagiana respectivamente a um, três e cinco centímetros da transição esôfago-gástrica (linha Z). O material foi enviado em meio de cultura aos laboratórios de microbiologia e de anatomia patológica para a identificação dos germes presentes. Posteriormente, os resultados foram correlacionados ao grau de evolução do megaesôfago segundo a classificação de Ferreira-Santos. Observou-se que é alta a incidência de germes patogênicos no megasôfago, independentemente do grau de dilatação, transformando a cirurgia para o tratamento desta afecção em potencialmente contaminada. Não houve diferença significativa quanto à positividade das culturas em relação ao grau de dilatação esofágica.

Abstract in English:

Chagasic patients with megaesophagus were submitted to an endoscopy of the upper digestive tract and the samples were collected with special instruments under sterilized conditions. One of the four samples collected was from the stase liquid and the other three samples were collected from fragments of the esophageal mucosa at one, three and five centimeters from the esophageal-stomach transition (Z line). The samples were analized by the Microbiology and Pathologic labs for the identification of microorganisms. After that, the results were correlated with the degree of mega esophagus according to Ferreira-Santos. We observed that the incidence of pathogenic microorganism is very high in megaesophagus, with no relation with the degree of dilatation making the surgery for the treatment of this afection potencially contaminated. There was no significant difference concerning the positivity of the culture relating to the degree of esophagus dilatation.

Recommendations for the treatment of bancroftian filariasis in symptomless and diseased patients Dreyer, Gerusa Dreyer, Patrícia Norões, Joaquim

Abstract in Portuguese:

As bases para se instituir o tratamento da filariose linfática são: prevenir, reverter ou diminuir a progressão da doença e interromper a transmissão do parasita. A escolha das medidas mais adequadas para o tratamento dos pacientes portadores de filariose linfática requer o conhecimento das diversas manifestações clínicas, assim como os seus diferentes mecanismos patogênicos. No passado, o tratamento da filariose era baseado somente na prescrição da droga antifilarial, porém, para os pacientes apresentando as manifestações agudas ou crônicas da doença, é imperativo que outras medidas sejam indicadas, incluindo higiene e dieta. Assim, essa nova visão multidisciplinar terapêutica é tão ou mais importante do que o início imediato da terapia antifilarial. Independentemente da manifestação clínica do paciente, três componentes estão agora indicados: 1) os cuidados clínicos e higiênico-dietéticos específicos para as diversas manifestações clínicas, 2) a educação e o suporte psicológico e finalmente 3) o tratamento quimioterápico com a dietilcarbamazina (DEC) ou a combinação de DEC e ivermectina. Os autores descrevem também as eficácias relativas macro e microfilaricidas da dietilcarbamazina e da ivermectina sozinhas ou em combinação usadas em programas de tratamento em massa para interrupção da transmissão e comentam sobre a introdução do uso da higiene em programas de saúde pública para a prevenção do linfedema.

Abstract in English:

The goals of treatment for lymphatic filariasis are: to prevent, reverse, or halt progression of disease; and to interrupt transmission of the parasite. Selecting the appropriate therapy for the patient with lymphatic filariasis requires knowledge of the various clinical features of filarial disease and their pathogenesis. In the past, treatment of lymphatic filariasis has focused primarily on antiparasitic chemotherapy; however, for many of the acute and chronic manifestations of lymphatic filariasis, it is now clear that supportive or other forms of clinical care are even more important than antiparasitic medication in order to prevent worsening of the disease. Regardless of the clinical manifestations of filarial disease in a particular patient, the following three components of treatment should, in general, be considered: supportive or diseasespecific clinical care (including higiene and diet), patient education and counseling and finally, antiparasitic chemotherapy with diethylcarbamazine (DEC) and/or the combination of DEC with ivermectin. The authors also describe the proportional efficacy of diethylcarbamazine and ivermectin, alone or in combination, for use in mass treatment aiming at transmission interruption and the use of hygiene as a public health approach for lymphedema prevention.

Neste artigo, o autor atualiza dados epidemiológicos sobre a tuberculose e descreve a luta contra a doença no Brasil e no mundo. Destaca aspectos históricos sobre as políticas do controle da doença no país e examina o Plano Nacional de Controle da Tuberculose ora vigente. Termina assinalando alguns problemas pendentes e discute novas alternativas para o controle da tuberculose.

Epidemiological data on tuberculosis and the fight against the disease in Brazil and in the world are reviewed herein. Historical aspects of the different approaches to the control of the disease in Brazil are highlighted and the ongoing Brazilian Program for the Control of Tuberculosis is examined. To conclude, the author takes a look at unresolved problems and considers new alternatives for the control of tuberculosis.

O desenvolvimento de piora clínica paradoxal como resposta ao início do tratamento da tuberculose pode complicar a evolução de pacientes com e sem infecção pelo HIV. Apresentamos uma grave manifestação da chamada reação paradoxal ao tratamento da tuberculose, que revelou doença meníngea previamente silenciosa em um paciente HIV-positivo de 34 anos.

The development of paradoxical clinical worsening following initiation of tuberculosis treatment may complicate the clinical course of both HIV-infected and uninfected patients. We report a severe manifestation of the so called paradoxical reaction to the treatment of tuberculosis that unmasked previously silent meningeal disease in a 34-year-old HIV-infected male patient.

Há poucas descrições de associação entre megacólon chagásico e câncer de cólon. Relatamos caso de abdômen obstrutivo por adenocarcinoma do cólon esquerdo, em megacólon chagásico. A revisão da literatura indica 8 casos desta associação e, analisando-se em conjunto as séries de levantamentos de câncer em megas chagásicos, encontra-se freqüência de 4,8% em megaesôfago e 0,1% em megacólon.

Abstract in English:

There are few descriptions of association between chagasic megacolon and colon cancer. We report a case of obstructive abdomen caused by adenocarcinoma of the left colon in chagasic megacolon. A review of the literature revealed 8 cases of this association and, analyzing together the series of findings of cancer in chagasic organomegalies, we found a frequency of 4.8% in megaesophagus and 0.1% in megacolon.

Testes suplementares para melhorar a especificidade do anti-VHC por ELISA nos bancos de sangue não são oficialmente recomendados no Brasil. No intuito de avaliar a taxa de falso-positivos, 70 doadores com transaminases normais e anti-VHC por ELISA foram submetidos a imunoblot de 3ª geração no Hemocentro de Mato Grosso, que não dispõe da técnica da reação de cadeia de polimerase. O teste confirmou o anti-VHC em 44 (62,9%), sendo negativo em 22 (31,4%) e indeterminado em 4 (5,7%). Confirmação pelo imunoblot ajuda a identificar os testes ELISA que são falso-positivos, tranqüilizando o grande contingente de doadores nessa situação e separando os que necessitam de acompanhamento médico. Com esse objetivo, sugere-se que o imunoblot poderia ser útil nos bancos de sangue brasileiros que não contam com técnicas de Biologia Molecular.

Supplemental tests using Immunoblot are recommended to improve specificity of anti-HCV by ELISA. In Brazil immunoblot is not officially recommended. Aiming to identify EIA false-positive rate 70 positive EIA anti-HCV blood donors were submitted to 3rd generation immunoblot at Hemocentro of Mato Grosso State where polymerase chain reaction tests are not performed. There were 44 (62.9%) immunoblot-positive, 22 (31.4%) negative and 4 (5.7%) indeterminate. Anti-HCV immunoblot can distinguish blood donors with false-positive ELISA from those who need medical assessment. Our data suggest that immunoblot could be useful in Brazilian blood banks where molecular biology tests are not available.
