RESUMO Introdução: A obstrução nasofaríngea pode alterar o crescimento craniofacial além de provocar impacto psicossocial e na qualidade de vida. Dessa forma é importante identificar o potencial de terapias ortodônticas em alterar as dimensões das vias aéreas. Objetivos: Avaliar as alterações em longo prazo nas dimensões das vias aéreas faríngeas em pacientes com Classe II esquelética tratados com aparelho extrabucal de puxada cervical. Material e Métodos: Foi realizado um estudo retrospectivo com 20 pacientes atendidos por um único ortodontista experiente. O espaço aéreo foi avaliado por meio de medidas lineares e de área em radiografias cefalométricas pré-tratamento, pós-tratamento e em pós-contenção de 25 anos. Resultados: As medidas apresentaram concordância elevada a moderada. Nenhuma das medidas apresentou diminuição após o tratamento. As áreas nasofaríngea, orofaríngea e hipofaríngea não foram modificadas após a intervenção com o aparelho extrabucal e mostraram uma tendência a estreitar ao longo do período de acompanhamento pós-tratamento. Em relação às medidas lineares, as dimensões nasofaríngeas permaneceram inalteradas ou aumentaram após o tratamento, retornando às dimensões iniciais na avaliação de seguimento. Conclusão: O tratamento ortopédico com aparelhos extraorais não provocou diminuição da dimensão das vias aéreas faríngeas e é considerado seguro para o tratamento de pacientes em crescimento com má oclusão esquelética de Classe II, de acordo com os resultados do presente estudo.

ABSTRACT Introduction: Nasopharyngeal obstruction may influence craniofacial growth, lead to psychosocial impairments, and adversely affect quality of life. Thus, it is important to identify the potential of orthodontic therapies to alter the dimensions of the airways. Objectives: To evaluate the long-term changes in pharyngeal airway dimensions in skeletal Class II patients treated with cervical headgear appliance. Material and Methods: A retrospective study was conducted with 20 patients treated by an experienced Orthodontist. The airway space was assessed using linear and area measurements on cephalometric radiographs before treatment, after treatment, and at a 25-year follow-up. Results: measurements showed high to moderate reliability. None of the measures showed a decrease after treatment. The nasopharyngeal, oropharyngeal, and hypopharyngeal areas were not modified after headgear intervention and showed a tendency to narrow over the follow-up period. Regarding linear measurements, the nasopharyngeal dimensions remained unchanged or increased after treatment, returning to initial dimensions in the follow-up assessment. Conclusion: Orthopedic treatment with extraoral appliances did not cause a decrease in pharyngeal airway dimension and is considered safe for the treatment of growing skeletal Class II malocclusions according to the results of the present study.

RESUMO Introdução: O objetivo do presente estudo foi avaliar a precisão do diagnóstico das lupas dentárias autofluorescentes Reveal para detectar biofilme, desmineralização e cárie ao redor de braquetes ortodônticos. Métodos: Foram selecionados 60 pacientes, sendo 30 pacientes sem tratamento ortodôntico (grupo controle) e 30 pacientes submetidos a tratamento ortodôntico (grupo experimental). Os 30 pacientes sem tratamento ortodôntico foram avaliados clinicamente por dois avaliadores previamente calibrados, que avaliaram a presença de biofilme, desmineralização e cárie incipiente na superfície vestibular dos dentes anteriores superiores por meio de dois métodos: G1 (n=30) - método visual; G2 (n=30) - método Reveal®. Os 30 pacientes com tratamento ortodôntico foram avaliados da mesma forma. Em ambos os grupos, foi atribuído um sistema de pontuação para avaliar as variáveis do estudo. Para a análise estatística, foi calculado um valor de escore médio para cada paciente, somando os escores de cada dente e dividindo pelo número de dentes avaliados. A comparação estatística foi realizada pelo teste Wilcoxon Signed Ranks (α=0,05). Resultados: O método Reveal foi mais eficaz na detecção da presença de biofilme nos pacientes sem tratamento ortodôntico (p<0,05), sem diferenças estatisticamente significativas em relação ao método visual na detecção de desmineralização e cárie (p>0,05). Além disso, o método Reveal foi mais eficiente na detecção da presença de biofilme e desmineralização nos pacientes com tratamento ortodôntico sem diferenças estatisticamente significativas em relação ao método visual na detecção de cáries (p>0,05). Conclusões: As lupas dentárias autofluorescentes Reveal foram eficazes na detecção de biofilme, desmineralização e cáries incipientes ao redor de braquetes ortodônticos.

ABSTRACT Introduction: The aim of the present study was to evaluate the diagnostic accuracy of Reveal autofluorescent dental loupes for detecting biofilm, demineralization, and caries around orthodontic brackets. Methods: Sixty patients were selected, being thirty patients without orthodontic treatment (control group) and thirty patients undergoing orthodontic treatment (experimental group). The 30 patients without orthodontics treatment were clinically evaluated by two previously calibrated evaluators, who assessed the presence of biofilm, demineralization, and incipient caries on the buccal surface of the upper anterior teeth by using two methods: G1 (n=30)-visual method; G2 (n=30)-Reveal® method. The 30 patients with orthodontic treatment were evaluated in the same way. In both groups, a scoring system was assigned to evaluate the variables of the study. For statistical analysis, an average score for each patient was calculated by adding the scores for each tooth and dividing by the number of teeth evaluated. A statistical comparison was performed by the Wilcoxon signed-ranks test (α = 0.05). Results: The Reveal method was more effective at detecting biofilm in patients without orthodontic treatment (p < 0.05). There were no statistically significant differences between the Reveal method and the visual method in detecting demineralization and caries (p > 0.05). Furthermore, the Reveal method was more efficient in detecting biofilm and demineralization in patients with orthodontic treatment (p < 0.05) and showed no statistically significant differences compared to the visual method in detecting caries (p > 0.05). Conclusions: The Reveal autofluorescent dental loupes effectively detected biofilm, demineralization, and incipient caries around orthodontic brackets.

Effect of print layer height on the accuracy and fit of the rapid-prototyped retainers GUPTA, Pranita CHAUDHARI, Prabhat Kumar DUGGAL, Ritu SAMRIT, Vilas D

RESUMO Introdução: O objetivo deste estudo foi avaliar comparativamente a precisão e o ajuste de contenções transparentes de prototipagem rápida impressos em duas alturas de camada diferentes (100 µm e 50 µm) usando uma impressora 3D de estereolitografia de baixa força (LFS). Métodos: Quarenta e duas contenções foram projetadas digitalmente usando o software de design auxiliado por computador (CAD) Maestro 3D Dental (AGE Solutions, Pisa, Itália) e impressas por prototipagem rápida usando uma impressora LFS (Formlabs Inc, Somerville, MA, EUA). Metade das contenções (21) foram impressas em uma altura de camada de 50 µm e a outra metade, em 100 µm. Para avaliar a precisão dimensional, medições lineares foram feitas nas contenções impressas em 3D usando um paquímetro digital (com precisão de 0,01 mm) e nas contenções projetadas digitalmente usando o software Maestro 3D Dental Studio. O ajuste das contenções foi avaliado posteriormente com um microscópio eletrônico de varredura. A precisão foi comparada entre três grupos, contenções projetadas digitalmente e aquelas impressas em alturas de camada de 50 µm e 100 µm, usando os testes ANOVA/Kruskal-Wallis. A confiabilidade e a consistência interna das medidas lineares foram avaliadas pelo coeficiente de correlação intraclasse (ICC com IC de 95%) e pelo alfa de Cronbach, respectivamente. Testes t pareados foram utilizados para comparar o ajuste de contenções impressas por prototipagem rápida, impressas em camadas de 100 µm e 50 µm. Além disso, o tempo de impressão de cada contenção foi registrado. Resultados: Não houve diferença estatisticamente significativa (valor de p > 0,05) entre a precisão das contenções impressas em 3D nas alturas de camada de 100 µm e 50 µm, com alta confiabilidade (ICC > 0,9) e alta consistência interna (alfa de Cronbach > 0,9). Na avaliação comparativa do ajuste, não foi observada diferença estatisticamente significativa (p > 0,05) entre as contenções impressas nas alturas de camada de 50 µm e 100 µm, exceto na borda labiogengival e no meio da borda incisal do incisivo central. O tempo necessário para a impressão das contenções na altura de 50 µm é maior do que o da altura da camada de 100 µm. Conclusão: Ambas as contenções de prototipagem rápida, impressas em camadas de 100 µm e 50 µm, se mostraram precisas para uso clínico, quando comparadas às contenções digitais de referência, com ajuste aceitável. Como o tempo necessário para imprimir contenções a 100 µm é menor do que as de camadas de 50 µm, é melhor usar camadas de 100 µm para a prototipagem rápida das contenções.

ABSTRACT Introduction: The purpose of this study was to comparatively evaluate the accuracy and fit of rapid-prototyped clear retainers printed at two different layer heights (100 µm and 50 µm) using a low-force stereolithography (LFS) 3D printer. Methods: Forty-two retainers were digitally designed using Maestro 3D Dental computer aided designing (CAD) software (AGE Solutions, Pisa, Italy) and rapid prototyped using an LFS printer (Formlabs Inc, Somerville, MA, USA). Half of the retainers (21) were printed at 50 µm layer height and other half at 100 µm. To assess the dimensional accuracy, linear measurements were taken from the 3D printed retainers using digital caliper (accurate to 0.01 mm) and from the digitally designed retainers using Maestro 3D Dental Studio software. The fit of the retainers was further evaluated with a scanning electron microscope. Accuracy was compared across three groups-digitally designed retainers, and those printed at 50 µm and 100 µm layer height using ANOVA/Kruskal‒Wallis tests. The reliability and internal consistency of the linear measurements were evaluated by the intraclass correlation coefficient (ICC with 95% CI) and Cronbach’s alpha, respectively. Paired t-tests were used to compare the fit of rapid prototyped retainers printed at 100 µm and 50 µm layer heights. Additionally, the printing time for each retainer was recorded. Results: There was no statistically significant difference (p value > 0.05) between the accuracy of 3D-printed retainers printed at 100 µm and 50 µm layer heights with a high reliability (ICC>0.9) and high internal consistency (Cronbach’s alpha>0.9). For comparative evaluation of the fit, no statistically significant difference (p>0.05) was observed among the retainers printed at 50 µm and 100 µm layer heights, except at the labio-gingival border and at the middle of the incisal edge of the central incisor. Time required for printing retainers at 50 µm is more than that of 100 µm layer height. Conclusion: Both rapid-prototyped retainers printed at 100 µm and 50 µm layer heights were found to be accurate for clinical use when compared to reference digital retainers with acceptable fit. As the time required for printing retainers at 100 µm is less than that of 50 µm layer height, it is better to use 100 µm layer height for rapid prototyping of the retainers.

RESUMO Objetivo: O objetivo do presente estudo foi avaliar as alterações volumétricas tridimensionais no espaço da articulação temporomandibular (ATM) após cirurgia de avanço mandibular em casos de Classe II esquelética com mandíbulas retrognáticas. A hipótese nula para este estudo foi a de que não haveria alterações estatísticas após a cirurgia ortognática. Material e métodos: Foi realizado um estudo retrospectivo com 30 documentações contendo tomografia computadorizada de feixe cônico (TCFC) de pacientes com Classe II esquelética com mandíbulas retrognáticas, tratados com osteotomia sagital bilateral (BSSO) para avanço mandibular de 6 a 8 mm. Avaliações volumétricas do espaço articular foram realizadas no pré-operatório (T0), uma semana após a cirurgia (T1) e 12 meses após a cirurgia (T2). Resultados: O espaço articular anterior médio apresentou um aumento de 612,37 mm3 em T0 a 876,67 mm3 em T1, seguido de uma ligeira redução para 862,4 mm3 em T2. Por outro lado, o espaço articular posterior diminuiu de 861 mm3 em T0 para 736,27 mm3 em T1 e posteriormente para 653,07 mm3 em T2. O espaço articular medial sofreu um aumento de 525,83 mm3 em T0 para 850,93 mm3 em T1, antes de diminuir para 596,77 mm3 em T2. O espaço articular lateral apresentou diminuição de 951 mm3 em T0 para 762 mm3 em T1, aumentando posteriormente para 918,7 mm3 em T2. Coeficientes de correlação fortemente positivos (aproximando-se de 1) foram observados na maioria das comparações T1 e T2 (p < 0,05). Conclusão: Alterações volumétricas significativas foram observadas no espaço da ATM após cirurgia de avanço mandibular nos casos de retrognatismo. As alterações se mantiveram mesmo 12 meses após a cirurgia, com uma leve recidiva.

Abstract in English:

ABSTRACT Objective: The aim of the present study was to assess three-dimensional volumetric changes in the temporomandibular joint (TMJ) space after mandibular advancement surgery in skeletal Class II cases with retrognathic mandibles. The null hypothesis for this study posited that there would be no statistical changes after orthognathic surgery. Materials and methods: A retrospective study was conducted on 30 cone-beam computed tomographic (CBCT) records of skeletal Class II patients with retrognathic mandibles who were treated with bilateral sagittal split osteotomy (BSSO) for mandibular advancement of 6-8 mm. Volumetric assessments of joint space were performed pre-operative (T0), one-week post-operative (T1), and 12-month post-operatively (T2). Results: The mean anterior joint space exhibited an increase from 612.37 mm3 at T0 to 876.67 mm3 at T1, followed by a slight reduction to 862.4 mm3 at T2. Conversely, the posterior joint space diminished from 861 mm3 at T0 to 736.27 mm3 at T1 and further to 653.07 mm3 at T2. The medial joint space experienced an increase from 525.83 mm3 at T0 to 850.93 mm3 at T1, before declining to 596.77 mm3 at T2. The lateral joint space showed a decrease from 951 mm3 at T0 to 762 mm3 at T1, subsequently rising to 918.7 mm3 at T2. Strong positive correlation coefficients (approaching 1) were observed in the majority of the T1 and T2 comparisons (p < 0.05). Conclusion: Significant volumetric changes were observed in the TMJ space after mandibular advancement surgery for retrognathism. The changes were maintained even 12 months after surgery, with a slight relapse.

RESUMO Introdução: A inteligência artificial (IA) na saúde aumentou suas aplicações nos últimos anos. A grande quantidade de dados disponível, devido ao aprimoramento do armazenamento e dos exames digitais, proporcionou um melhor conhecimento no planejamento do tratamento e abriu novas possibilidades de aplicação da IA no diagnóstico e planejamento ortodôntico. Objetivo: O objetivo desta revisão de escopo foi examinar quando os modelos de IA aprimoram o processo de tomada de decisão clínica no diagnóstico e planejamento ortodôntico, com foco na utilização de imagens radiográficas. Métodos: Estratégias eletrônicas de busca individual foram desenvolvidas e realizadas nas bases PubMed/Medline, Scopus, Web of Science, Embase, Lilacs e Cochrane Library (somente artigos em inglês publicados de janeiro de 2000 a 20 de outubro de 2021), buscando estudos relevantes que atendessem aos critérios de elegibilidade. Resultados: Doze estudos foram incluídos, categorizados em cinco grupos distintos: cirurgia ortognática, osteoartrite da articulação temporomandibular (ATM), padrão esquelético, apneia obstrutiva do sono (AOS) e maturação/desenvolvimento esquelético. A maioria dos modelos de IA utilizados baseava-se em aprendizado profundo (Deep Learning - DL), sendo as radiografias laterais o tipo de imagem mais utilizado. Conclusão: A integração da IA na prática clínica provavelmente continuará a evoluir, aprimorando o planejamento do tratamento em casos seletivos. As melhores aplicações foram observadas na osteoartrite da ATM, maturação esquelética, classificação esquelética, AOS e necessidade de cirurgia ortognática.

ABSTRACT Introduction: Artificial intelligence (AI) in health has increased its applications over the last years. The large amount of data available due to the improvement of data storage and digital exams provided better knowledge in treatment planning and opened new possibilities of applications of AI in Orthodontics’ diagnosis and treatment planning. Objective: The aim of this scoping review was to examine when AI models enhance the clinical decision-making process in orthodontic diagnosis and treatment planning, with a focus on the utilization of X-ray-based imaging. Methods: Individual eletronic search strategies were developed and conducted on PubMed/Medline, Scopus, Web fo Science, Embase, Lilacs, and Cochrane Library (only English language articles published from January 2000 to October 20, 2021), aiming for relevant studies that met the eligibility criteria. Results: 12 studies were included, categorized in 5 different groups: Orthognathic surgery, Temporomandibular Joint (TMJ) osteoarthritis, skeletal pattern, obstructive sleep apnea (OSA), and skeletal maturation/development. Most of the AI models used were Deep Learning (DL) based and the X-ray image that was most used was lateral radiographs. Conclusion: Integrating AI into clinical practice will likely continue to evolve, enhancing treatment planning in selective cases. The best applications were over TMJ osteoarthritis, skeletal maturation, and classification, OSA, and the need for orthognathic surgery.

RESUMO Objetivo: O objetivo do presente estudo foi investigar a relação entre o padrão de abertura bucal e o padrão esquelético facial em uma população do sul da Itália. Materiais e Métodos: Este estudo retrospectivo incluiu 415 indivíduos. O padrão esquelético sagital foi classificado em Classe I, II e III, de acordo com o ângulo ANPg, e o padrão de crescimento vertical foi classificado como hipodivergente, normodivergente e hiperdivergente, com base no ângulo SNGoGn. Os registros de abertura bucal máxima (ABM) e os valores correspondentes a 20% e 80% dessa distribuição na curva normal foram considerados para fins de análise. Resultados: A mediana de idade foi de 23 anos (intervalo interquartil: 20-28), e a média da ABM foi de 52,1 mm (DP: 6). Houve diferença estatisticamente significativa na abertura bucal em relação ao sexo (P = 0,01) e ao padrão de crescimento vertical (P = 0,02). O teste post hoc (Tukey) demonstrou que homens (53,6 mm; DP: 6,43) apresentam maior abertura bucal do que mulheres (51,4 mm; DP: 5,72), e pacientes hiperdivergentes (50,6 mm) apresentam menor abertura bucal do que os hipodivergentes (52,8 mm) e normodivergentes (52,5 mm). Os testes ANOVA de dois fatores, realizados para os padrões de abertura bucal baixa e alta, confirmaram os achados prévios da amostra geral. Conclusões: O sexo e o padrão de crescimento vertical parecem influenciar a amplitude da abertura bucal.

Abstract in English:

ABSTRACT Objective: The aim of the present study was to investigate the relationship between mouth opening pattern and facial skeletal pattern in a southern Italian population. Materials and Methods: This retrospective study included 415 subjects. The sagittal skeletal pattern was classified in Class I, II, and III according to ANPg angle and the vertical growth pattern was classified as hypodivergent, normodivergent, and hyperdivergent according to SNGoGn angle. Maximum mouth opening (MM0) records and 20% and 80% of these values in the normal distribution curve were considered for analysis purposes. Results: The median age was 23 years (IQR, 20-28), and the mean MMO was 52.1 mm (SD: 6). There was a statistical difference regarding the mouth opening according to sex (P:0.01) and vertical growth pattern (P: 0.02). The post hoc test (Tukey) showed men (53.6 mm) (SD 6.43) open the mouth more than women (51.4 mm) (SD 5.72), and hyperdivergent patients (50.6 mm) open less than hypodivergent (52.8 mm) and normodivergent (52.5 mm). The 2-way ANOVA tests regarding the low and high mouth opening patterns confirmed the previous findings of the general sample. Conclusions: sex and vertical growth pattern seem to influence the amount of mouth opening.

RESUMO Introdução: Expansão rápida da maxila (ERM) é um procedimento ortopédico usado para corrigir deficiências transversais maxilares pela aplicação de forças por meio de aparelhos especializados. Essa intervenção induz alterações dentárias e esqueléticas, que podem potencialmente provocar respostas fisiológicas de estresse. Objetivo: Validar um modelo para estimar o estresse medindo os níveis plasmáticos de cortisol em camundongos submetidos à ERM rápida. Métodos: Camundongos machos de 5 a 6 semanas de idade (C57BL6/J) foram submetidos à ERM realizada por um distrator de alça de abertura na sutura palatina mediana calibrada da seguinte forma (n = 5 por grupo): sem força (controle); 0,28 Newtons (N), 0,42N e 0,56N por períodos de 7 e 14 dias. Análises de histomorfometria foram realizadas para analisar o efeito da ERM e amostras de soro foram coletadas para medir o cortisol por ensaio imunoenzimático (ELISA). Resultados: As forças aplicadas em dois momentos diferentes resultaram em um padrão de ERM bem-sucedido caracterizado pela abertura da sutura palatina mediana em comparação ao grupo controle (P<0,05). A força de 0,42N em 7 dias resultou em um aumento significativo (P<0,05) no cortisol (pg/ml) em comparação aos grupos controle e 0,28N em 7 dias (d); o nível de cortisol no grupo 0,42N 7d foi estatisticamente maior quando comparado ao grupo 0,42N 14d. Não houve diferença estatisticamente significativa entre os outros grupos. Conclusão: Os resultados sugerem um aumento na resposta ao estresse durante os primeiros dias após a aplicação da força de ERM com uma força de 0,42N, com subsequente adaptação corporal.

ABSTRACT Introduction: Rapid maxillary expansion (RME) is an orthopedic procedure used to correct maxillary transverse deficiencies by applying forces through specialized appliances. This intervention induces both dental and skeletal changes, which may potentially elicit physiological stress responses. Objective: To validate a model for estimating stress by measuring plasma cortisol levels in mice subjected to rapid RME. Methods: 5-6-week-old male mice (C57BL6/J) were submitted to RME performed by an opening loop distractor at the mid palatal suture calibrated as follows (n = 5 per group): no force (control); 0.28 Newtons (N), 0.42N and 0.56N for periods of 7 and 14 days. Histomorphometry analyses were performed to analyze RME effect and serum samples were collected to measure cortisol by enzyme-linked immunosorbent assay (ELISA). Results: The forces applied at two different time points resulted in a successful RME pattern with the opening of the mid-palatal suture compared to the control group (P<0.05). The 0.42N force at 7 days resulted in a significant (P<0.05) increase in cortisol (pg/ml) compared to the control and 0.28N at 7 days (d) groups; the cortisol level in the 0.42N 7d group was statistically reduced when compared to the 0.42N 14d group. There was no statistically significant difference between the other groups. Conclusion: The results suggest an increase in the stress response during the first days after the application of RME force with a force of 0.42N, with subsequent body adaptation. The force parameter of 0.42N at days proved to be a valid model for analyzing RME in mice.

RESUMO Introdução: Apesar de seu apelo estético e crescente popularidade, os alinhadores enfrentam limitações significativas na realização de movimentos ortodônticos complexos, como extrusão anterior, rotações severas e distalização eficiente dos molares. Esses desafios biomecânicos frequentemente resultam em maior duração do tratamento e menor previsibilidade. Objetivo: O presente artigo explora a abordagem híbrida, que combina a eficiência biomecânica dos aparelhos fixos na fase inicial com as vantagens estéticas e de higiene dos alinhadores na fase de finalização. Os aparelhos fixos são utilizados para controlar movimentos que os alinhadores não executam com previsibilidade, como controle radicular, extrusão com proclinação e grandes rotações. Após essas correções, os alinhadores são empregados para finalizar o tratamento com maior conforto e aceitação por parte do paciente. Conclusão: Ao integrar estrategicamente ambos os sistemas, a terapia híbrida melhora a eficiência clínica, reduz o tempo total de tratamento e proporciona um atendimento personalizado, alinhado às expectativas do paciente moderno.

ABSTRACT Introduction: Despite their aesthetic appeal and rising popularity, clear aligners face significant limitations in performing complex orthodontic movements such as anterior extrusion, severe rotations, and efficient molar distalization. These biomechanical challenges often result in longer treatment durations and reduced predictability. Objective: This article explores the hybrid treatment approach, which combines the biomechanical efficiency of fixed appliances in the initial phase with the aesthetic and hygienic advantages of aligners in the finishing phase. Fixed appliances are employed to manage movements that aligners struggle to control reliably, including root torque, vertical extrusion with proclination, and large rotational corrections. Once these key adjustments are made, aligners are introduced to complete treatment with greater comfort and patient satisfaction. Conclusion: By strategically integrating both systems, hybrid therapy enhances clinical efficiency, reduces total treatment time, and delivers personalized care aligned with modern patient expectations.
