RESUMO JUSTIFICATIVA E OBJETIVOS: A identificação dos locais de dor é um aspecto essencial no manejo de pacientes com Dor Espalhada. Vários instrumentos foram desenvolvidos, incluindo desenhos de dor, um sistema de grade e métodos computadorizados. No entanto, ainda não se sabe se o Índice de Dor Espalhada coincide com um método automatizado (painMAP) para quantificar o número de áreas de dor. Portanto, este estudo teve como objetivo identificar a relação entre o Índice de Dor Espalhada e o painMAP para medir as áreas doloridas em participantes com esse quadro de dor. MÉTODOS: Uma análise secundária pré-planejada de dados coletados de 311 pacientes com dor musculoesquelética foi realizada. O Índice de Dor Espalhada e o painMAP avaliaram as áreas de dor. O coeficiente de correlação de Spearman foi utilizado para investigar a correlação entre o Índice de Dor Espalhada e o software painMAP. RESULTADOS: Um total de 98 participantes com Dor Espalhada foram incluídos neste estudo. A maioria dos participantes era do sexo feminino (67;83,7%), com média de idade de 57,7±11,5 anos, média de altura de 1,6 (0,1) metros e média de peso de 73,2 (11,8) quilogramas. A média de intensidade da dor foi de 6,7 (2,0) e da duração da dor de 92,3 (96,3) meses. O número médio de áreas de dor no Índice de Dor Espalhada foi de 10,1(3,7) e no software painMAP foi de 11,7 (8,8). Uma correlação positiva fraca (rho=0,26, IC de 95% 0,45-0,04, p=0,022) entre o Índice de Dor Espalhada e o painMAP foi encontrada. CONCLUSÃO: O Índice de Dor Espalhada e o painMAP mostraram correlação positiva fraca para avaliar as áreas de dor em participantes com dor espalhada.

ABSTRACT BACKGROUND AND OBJECTIVES: Identifying pain sites is essential to managing patients with Widespread Pain. Several instruments have been developed, including pain drawings, a grid system and computerized methods. However, it is not yet known whether the Widespread Pain Index matches an automated method (painMAP) for quantifying the number of pain areas. Therefore, this study aimed to identify the relationship between the Widespread Pain Index and the painMAP software to measure pain sites in participants with Widespread Pain. METHODS: A pre-planned secondary analysis of data collected from 311 patients with musculoskeletal pain was conducted. The Widespread Pain Index and the painMAP software assessed pain sites. Spearman’s correlation coefficient investigated the correlation between the Widespread Pain Index and the painMAP software. RESULTS: A total of 98 participants with Widespread Pain were included in this study. Most participants were female (67; 83.7%), with a mean age of 57,7±11,5 years, mean height of 1.6 (0.1) meters and mean weight of 73.2 (11.8) kilograms. The mean pain intensity was 6.7 (2.0), and the pain duration was 92.3 (96.3) months. The mean number of pain sites in the Widespread Pain Index was 10.1 (3.7), and in the painMAP software, it was 11.7 (8.8). A weak positive correlation (rho = 0.26, 95% CI 0.45 to 0.04, p = 0.022) between the Widespread Pain Index and the painMAP software was found. CONCLUSION: The Widespread Pain Index and the painMAP software showed a weak correlation for assessing pain sites in participants with Widespread Pain.

RESUMO JUSTIFICATIVA E OBJETIVOS: O objetivo do presente estudo foi avaliar a frequência dos subtipos de disfunção temporomandibular (DTM) em indivíduos que buscaram atendimento especializado, identificando os subtipos da DTM, se muscular e/ou articular, além de relacionar cada subtipo com variáveis como sexo e faixa etária. MÉTODOS: Neste contexto, após uma triagem, foram selecionados 270 indivíduos na cidade de Fortaleza/CE, com faixa etária entre 18 anos e 70 anos de idade. Todos os indivíduos foram avaliados por meio do instrumento Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). RESULTADOS: Entre os 267 pacientes, 88,7% (n = 237) foram do sexo feminino e 11,3% (n = 30) do masculino, com média de idade de 42 ± 11,8 anos. Neste estudo evidenciou-se a predominância de DTM articular/muscular (51,7%; n = 138), seguida de indivíduos com DTM somente muscular (47,5%; n = 127) e por último a articular (0,8%; n = 2). Não foram identificadas correlações importantes (p > 0,05), segundo o teste qui-quadrado, quando comparados os subtipos de DTM com as variáveis gênero e idade. Ao relacionar o diagnóstico com o subtipo de DTM, evidenciou-se que a mialgia bilateral foi a mais prevalente na DTM muscular (n = 100; 37,4%) e articular/muscular (n = 112; 41,9%). As demais variáveis não apontaram significância estatística nem correlação moderada ou forte. CONCLUSÃO: Nesta pesquisa, foram encontrados todos os tipos de DTM, havendo um claro predomínio da DTM do tipo articular/muscular, seguida da somente muscular, especialmente em indivíduos do gênero feminino e de idade entre 39 e 48 anos.

ABSTRACT BACKGROUND AND OBJECTIVES: The goal of the present study was to assess the frequency of temporomandibular disorders (TMD) subtypes in individuals that search for specialized care, identifying the subtypes of TMD, muscular and/or articular, in addition to relating each subtype with variables such as gender and age. METHODS: In this context, after a screening, 270 individuals, aged between 18 and 70 years, were selected. All data were acquired using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) instrument. RESULTS: Among the 267 patients, 88.7% (n = 237) were female and 11.3% (n = 30) were male, with a mean age of 42±11.8 years. In this study, the predominance of joint/muscle TMD was evidenced (51.7%; n = 138), followed by only muscle TMD (47.5%; n = 127) and lastly, joint TMD (0.8%; n = 2). Important correlations (p>0.05) were not identified when comparing TMD subtypes with the variables gender and age, according to chi-square test. By relating the diagnosis to the TMD subtype, it was evidenced that bilateral myalgia was the most prevalent in muscle TMD (n = 100; 37.4%) and articular/muscular (n = 112; 41.9%). The other variables did not show significant statistics, neither moderate nor strong correlation. CONCLUSION: In this research, all TMD subtypes were found, with a clear predominance of joint/muscle type of TMD, followed by muscle only, especially in females aged between 39 and 48 years.

Um único exercício pode ser usado como uma opção para as fases iniciais da reabilitação. O exercício resistido de baixa intensidade com ou sem restrição do fluxo sanguíneo, pode ser aplicado como alternativa em casos de osteoartrose do joelho.

HIGHLIGHTS There is no difference in pain improvement whether the exercise is performed with or without blood flow restriction. A single exercise can be used as an option to the early phases of rehabilitation. Low-intensity resistance exercise with or without blood flow restriction, can be applied as an alternative in cases of knee osteoarthritis.

DESTAQUES Pistas Indiretas Planas apresentaram um desempenho melhor do que as placas miorrelaxantes no tratamento da cefaleia secundária à disfunção temporomandibular. Pistas Indiretas Planas reduziram o número de dias com dor de cabeça, a intensidade da dor e o uso de analgésicos. Pistas Indiretas Planas podem ser uma alternativa viável para o controle da cefaleia.

HIGHLIGHTS Planas Indirect Tracks performed better than occlusal splints in the treatment of headache secondary to temporomandibular disorder. Planas Indirect Tracks reduced the number of days with headache, the intensity of pain and use of analgesics. Planas Indirect Tracks may be a viable alternative for headache control.

DESTAQUES Estudo sobre uma doença que está em crescimento no Brasil e no mundo. Estima-se que no Brasil cerca de 30 mil óbitos/ano acontecem devido à doença pulmonar obstrutiva crônica (DPOC), sendo esta a quinta maior causa de morte no país. Animais com DPOC induzida por instilação de elastase traqueal apresentam níveis elevados de marcadores inflamatórios. Animais com DPOC apresentam respostas alteradas frente ao estímulo nocivo, sugerindo uma influência dos marcadores inflamatórios na percepção nociceptiva.

HIGHLIGHTS This is a study about a disease that is on the rise in Brazil and in the world. It is estimated that in Brazil about 30 thousand deaths/year occur due to chronic obstructive pulmonary disease (COPD), being the fifth leading cause of death. Animals with COPD induced by tracheal elastase instillation have elevated levels of inflammatory markers. Animals with COPD show altered responses to noxious stimuli, suggesting influence of inflammatory markers on nociceptive perception.

DESTAQUES A educação em neurociência da dor é uma intervenção não farmacológica que deve ser oferecida ao indivíduo com dor musculoesquelética crônica. Existe uma lacuna na literatura acerca da qualidade dos serviços de saúde prestados na modalidade de teleatendimento comparado às consultas presenciais. Os participantes deste estudo mostraram boa satisfação e percepção de um programa de educação em neurociência da dor tanto na modalidade online quanto na modalidade presencial.

HIGHLIGHTS Pain neuroscience education is a non-pharmacological intervention that should be offered to the individual with chronic musculoskeletal pain. There is a gap in the literature about the quality of health care services provided over the phone compared to face-to-face consultations. The study’s participants showed good satisfaction and perception of a pain neuroscience education program in both online and face-to-face modalities.

RESUMO JUSTIFICATIVA E OBJETIVOS: Os exercícios domiciliares podem melhorar a função e a qualidade de vida de pacientes com dor no ombro. Conhecer facilitadores e barreiras é fundamental para adesão ao tratamento da dor no ombro. Acredita-se que indivíduos que aderem aos exercícios domiciliares apresentem poucas barreiras ambientais, baixa intensidade de dor e incapacidade do ombro. O objetivo deste estudo foi identificar facilitadores e barreiras para a adesão a um programa de exercícios domiciliares em indivíduos com dor no ombro e investigar a influência de barreiras ambientais, da dor e incapacidade no ombro, e da cinesiofobia na adesão ao programa. MÉTODOS: Neste estudo transversal, indivíduos com dor no ombro realizaram exercícios domiciliares por oito semanas com base em uma cartilha além da fisioterapia supervisionada ou como sua única terapia. Os indivíduos relataram facilitadores e barreiras percebidos para a adesão aos exercícios em casa e responderam ao Craig Hospital Inventory of Environmental Factors-Brasil (CHIEF-Br), Shoulder Pain and Disability Index-Brasil (SPADI-Br) e Escala Tampa de Cinesiofobia (TAMPA). Os escores do CHIEF-Br, SPADI-Br e TAMPA foram analisados como preditores de adesão por meio de modelos de regressão logística. RESULTADOS: Participaram do estudo 50 indivíduos e 88% aderiram ao programa. O facilitador e a barreira mais frequentes foram “receber orientação de um profissional” e “falta de tempo disponível”, respectivamente. Dor e incapacidade foram os únicos preditores de adesão (p=0,044), e o aumento de um ponto no SPADI-Br reduziu a probabilidade de adesão aos exercícios domiciliares em 14% (Exp(B) =0,86). CONCLUSÃO: Os indivíduos relataram facilitadores e barreiras ao programa de exercícios. A barreiras ambientais avaliadas por meio do CHIEF-Br e a cinesiofobia não foram preditores da adesão.

ABSTRACT BACKGROUND AND OBJECTIVES: Home-based exercises can improve function and quality of life in shoulder pain patients. Knowing the facilitators and barriers is crucial for adherence to shoulder pain treatment. It is believed that individuals who adhere to home exercises have fewer environmental barriers, pain intensity and shoulder disability. The aim of this study is to identify facilitators and barriers for adherence to a home-based exercise program in shoulder pain individuals, and to investigate the influence of environmental barriers, shoulder pain and disability, and kinesiophobia on adherence to a program. METHODS: This is a cross-sectional study. Shoulder pain individuals performed home-based exercises for eight weeks based on a booklet in addition to supervised physical therapy or as their sole therapy. They reported the perceived facilitators and barriers to home exercises adherence and answered the Craig Hospital Inventory of Environmental Factors (CHIEF), Shoulder Pain and Disability Index (SPADI), and the Tampa Scale for Kinesiophobia (TSK) questionnaires. Logistic regression models analyzed the scores of the CHIEF, SPADI, and TSK as predictors of adherence. RESULTS: Fifty individuals participated in this study and 88% adhered to the program. The most frequent facilitator and barrier were “having guidance from a professional,” and “lack of available time,” respectively. Pain and disability were the only predictor of adherence (p=0.044), and an increase of one point in SPADI reduced the likelihood of adherence to home exercises by 14% (Exp(B) =0.86). CONCLUSION: Individuals reported facilitators and barriers to the exercise program. Environmental barriers assessed using the CHIEF and kinesiophobia were not predictors of adherence.

A terapia farmacológica combinada com outras terapias potencializa as chances de controle de dores crônicas em DTM. O uso de canabinoides medicinais mostrou resultados promissores no manejo da dor crônica em pacientes com DTM.

HIGHLIGHTS Psychosocial changes act as amplifying factors for chronic pain in TMD and contribute negatively to its control. Pharmacological therapy combined with other therapies enhances the chances of controlling chronic TMD pain. The use of medicinal cannabinoids has shown promising results in the management of chronic pain in TMD patients.

DESTAQUES O laser de baixa intensidade (LBI) é um dos recursos mais utilizados na prática clínica para alívio da dor. Diante disso, é importante entender os seus reais efeitos no tratamento desse sintoma. O LBI parece apresentar bons resultados para alívio da dor, contudo há uma heterogeneidade entre os estudos que dificultam o entendimento real sobre esses efeitos. Necessita-se de mais ensaios clínicos robustos para se ter um melhor entendimento sobre a efetividade do LBI na diminuição da dor.

HIGHLIGHTS Low-level laser therapy (LLLT) is one of the most used resources in clinical practice for pain relief. Therefore, it is important to comprehend its real effects in the treatment of this symptom. LLLT seems to show good results for pain relief, however there is heterogeneity among studies that makes it difficult to really understand these effects. More robust clinical trials are needed to achieve a better comprehension of the LLLT effectiveness in reducing pain.
