Secondary late-onset Lennox-Gastaut syndrome: a critical view Barreira, Amilton Antunes Lison, Michel Pierre

De um grupo de 66 pacientes com sindrome de Lennox-Gastaut (SLG), selecionaram-se e se observaram 12 com manifestações sindrômicas se iniciando após o 6º ano de vida. Esses pacientes foram observados clínica e eletrencefalograficamente durante um período médio de 2,5 anos. Concluiu-se que a sindrome com início tardio pode: ocorrer após um longo intervalo entre a agressão encefálica difusa e as primeiras manifestações clínicas com ou sem alteração prévia do desenvolvimento psicomotor; associar-se, desde o início, a retardo mental grave; apresentar semiologia crítica (simples, complexa ou mista) semelhante à da forma precoce. Esses pacientes podem ainda apresentar: crises epilépticas complexas e mistas não referidas anteriormente na literatura; anormalidades EEG paroxísticas intercríticas que se sobrepõem às da forma precoce; no EEG complexos ponta-onda lenta que podem ser ativados pela hiperpnéia. Nossos resultados demonstram que a incidência da SLG após os 6 anos de idade, necessariamente não implica em menor freqüência de antecedentes orgânicos, em melhor desenvolvimento neuropsicomotor até o início da síndrome e na presença de maior proporção de crises não específicas (generalizadas ou parciais, principalmente as com sintomatologia elaborada). A expressão crítica e eletrencefalográfica da síndrome, secundária e se iniciando após o 6º ano de vida, poderia, pelo menos em parte, ser dependente da idade em que incidiu a agressão encefálica difusa.

Abstract in English:

From a group of 66 patients with the Lennox-Gastaut syndrome, 12 whose manifestations had started after the 6th year of life were selected for study. These patients were observed clinically and electroencephalographically for an average period of 2.5 years. We concluded that the late-onset syndrome can: occur after a long interval between diffuse encephalopathy and the first clinical manifestations, with or without previous alterations in psychomotor development; be associated from the onset with serious mental retardation; exhibit simple, complex and mixed seizures similar to those observed in the early form. These patients can also: suffer complex and mixed epileptic seizures previously unreported; paroxismal interictal EEG abnormalities that overlap those of the early form; and spike-slow wave complexes in the EEG that can be actived by hyperpnea. Our results demonstrate that the incidence of LGS after 6 years of age does not necessarily imply a lower frequency of organic antecedents, or beter neu-ropsychomotor development up to the onset of the syndrome or the presence of a higher rate of nonspecific seizures (generalized or partial seizures, and mainly those with elaborate symptomatolgy). The critical and encephalographic expression of the syndrome, which is secondary and starts after the 6th year of age, may depend at least in part on the age when diffuse encephalopathy started.

Fez-se o estudo anatomopatológico sistematizado do encéfalo de 31 casos da forma crônica cardíaca da doença de Chagas. Encontrou-se necrose paren-quimatosa eletiva em 64,5% dos casos afetando principalmente as células de Purkinje do cerebelo e alguns núcleos talâmicos, focos múltiplos de microne-crose antiga (51,6%) especialmente do córtex cerebral, enfartes cerebrais antigos (22,6%), necrose cortical em faixa (16,1%) e atrofia cortical cerebral (3,2%). Estes achados são inespecíficos e podem ser explicados em função de fenômenos trombo-embólicos e de hipoxemia decorrente de insuficiência cardiovascular bem como de distúrbios do ritmo cardíaco.

Abstract in English:

The pathological systematized study of the brain of 31 cases of the chronic cardiac form of Chagas' disease was undertaken. Elective parenchymatous necrosis was found in 64.5% of cases, mainly in cerebellar Purkinje cells, and in thalamic nuclei. Also found were multiple foci of old micronecrosis (51.6%) mostly in the cerebral cortex, old cerebral infarcts (22.6%), cortical laminar necrosis (16.1%) and cerebral cortical atrophy (3.2%). These findings are not specific and can be explained by thromboembolic phenomena and hypoxemia following from congestive heart failure and disturbances of the cardiac rhythm.
Computorized tomography in patients with increased intracranial pressure and obstructive hydrocephalus due to cerebral cysticercosts: comparison with Dimer-X ventriculography Colli, Benedicto Oscar Martelli, Nelson Assirati Jr, João Alberto Machado, Hélio Rubens Belluci, Angela

Estudo das alterações observadas nas tomografias computadorizadas (TCs) de 16 casos de neurocisticercose, com hipertensão intracraniana por hidrocefalia. Todos os casos foram submetidos a radiografias simples do crânio e 12, à ventriculografia com Dimer-X. Nas radiografias simples foram observados sinais de hipertensão intracraniana em 7, calcificações em um, e d foram normais. A TC mostrou parênquima cerebral normal em 9 casos e nos 7 restantes áreas hipodensas arredondadas de tamanho variado, com ou sem retenção de contraste na periferia, e/ou calcificações esparsas no parenquima. Hidrocefalia foi a única alteração observada no sistema ventricular nos casos em que o exame foi realizado previamente à derivação do LCR. As ventriculografias foram analisadas em trabalho anterior e permitiram localizar e determinar as características da obstrução e a presença de massas ocupando espaço dentro dos ventrículos. A comparação das ventriculografias e TCs realizadas na mesma época evidenciou que a primeira fornece melhores informações sobre o sistema ventricular mas, por outro lado, a tomografia fornece informações adicionais sobre o parênquima cerebral. A TC e a ventriculografia são exames que se completam na avaliação e no diagnóstico destes pacientes.

Abstract in English:

The authors reviewed the alterations observed on computorized tomography (CT) examinations of 16 patients with increased intracranial pressure and obstructive hydrocephalus due to cerebral cysticercosis. Plain radiograms of the skull were available in all cases and Dimer-X ventriculography in 12 cases. In 7 cases there were radiologic signs of increased intracranial pressure. CT scan showed normal cerebral parenchyma in 9 cases and low density areas, with various size, with or without surround contrast enhancement, and/or small calcifications in the parenchyma. Hydrocephalus was the only alteration observed in the ventricular system in the case that the examination was done before ventricular drainage. Ventriculography was analyzed in other paper and permitted to situate the obstruction, determine its morphological characteristics and identify occuping space lesions within the ventricles. Comparison between ventriculography and CT scan made in the same period evidenced that the former gives best information about the ventricular system and that tomography gives additional information about the cerebral parenchyma. In conclusion, CT scan and ventriculography are investigations that complete each the other for evaluation and diagnosis of cerebral cysticercosis.

Investigamos a resposta imunológica celular e humoral frente a extrato salino de tecido cerebral em 9 pacientes com síndrome de Lennox-Gastaut, 15 pacientes com síndrome de West e 20 crianças normais. A técnica de imunodifusão dupla em gel de agar (Ouchterlony) evidenciou em todos os pacientes, altos níveis de um anticorpo precipitante contra o extrato salino de tecido cerebral. O teste de inibição de migração de leucócitos com o mesmo antígeno mostrou-se positivo na maioria dos pacientes. O possível papel destas respostas autoimmunes na patogenia das sindromes de West e Lennox-Gastaut é discutido.

Abstract in English:

We investigated humoral and cellular immune response to brain tissues in 15 patients with West syndrome, in 9 patients with Lennox-Gastaut syndrome and in 20 healthy children. High levels of a precipitating antibody to a saline extract of brain tissue were detected in all patients; leucocyte migration inhibition test with the same antigen was found to be positive in most of them. The role of this autoimmune response in the pathogenesis of West and Lennox-Gastaut syndromes remains to the elucidated.

Investigamos a imunidade humoral e celular em 18 pacientes com sindrome de West, 12 com sindrome de Lennox-Gastaut e 19 controles. Os exames realizados foram os seguintes: percentagem de linfócitos Ô e  no sangue periférico, niveis séricos de IgG, IgA e IgM, sensibilização cutânea com o DNCB, PHA intra dérmica, teste de inibição de migração de leucócitos e transformação blástica de linfócitos em presença de PHA. Detectamos deficiência de imunidade celular em 28 crianças, (18 com sindrome de West e 10 com sindrome de Lennox-Gastaut) e baixos níveis de imunoglobulinas em apenas 6. A depressão imunitária mostrou-se mais intensa nas crianças que apresentam sindrome de West.

Cell-mediated and humoral immunity were investigated in 18 patients with West syndrome, 12 with Lennox-Gastaut syndrome and 19 healthy controls. The study included determination of Ô and  peripheral blood lymphocytes, serum levels of IgG, IgA and IgM, skin sensibilization with DNCB, intracutaneous PHA, leucocyte migration inhibition test and lymphocyte blastic transformation in the presence of PHA. Cell-mediated deficiency was detected in 28 children whereas low levels of immunoglobulins were observed only in 6 children. Immunological disturbances were more prominent in children with West syndrome.

Abstract in Portuguese:

O estudo citológico diferencial de amostras de LCR sub-aracnóideo cisternal ou lombar foi feito em 59 pessoas sem doença atual e com composição normal deste, líquido orgânico. Os preparados com as células foram obtidos com a câmara de sedimentação gravitacional facilitada de Reis. A finalidade deste trabalho foi confrontar estes resultados com aqueles já estabelecidos na literatura especializada. Um cuidado especial foi tomado para obter maior fidelidade de informação, o qual consistiu na enumeração de todas as células existentes no sedimento e não somente 100 células ou número menor, expresso em percentagem do total. O resultado desta pesquisa mostrou que o quadro citológico diferencial normal é constituído, em média, por 58,8% de células linfocitárias, 41,0% de células monocitóides e 0,2% de granulócitos neutrófilos. Estes números concordam com aqueles assinalados na literatura, na parte referente às proporções relativas de células linfocitárias e células monocitóides, porém revelam um fato raramente referido, que é a possibilidade da presença de raros granulócitos neutrófilos no quadro citológico diferencial normal do LCR.

Abstract in English:

Cisternal or lumbar cerebrospinal fluid (CSF) specimens of 59 healthy persons were examined for differential cell count with a new improved sedimentation chamber, similar to that one of Sayk. The purpose of this study was to compare our results to those already described in the literature. Techinical care was taken in order to increase the accuracy of this investigation and it consisted mainly of counting all the existing cells in the preparation, and not only 100 cells or a lesser number, recorded in percentage. The results of this survey have shown that the normal mean values for the CSF cytomorphology are 58.8 per cent lymphocitic cells, 41.0 per cent monocytoid cells, and 0.2 per cent neutrophilic granulocytes. These figures are in good agreement with those already reported by numerous authors, when we consider the lymphocytic and monocytoid cells, but they revealed a fact seldom seen reported: the possibility of a few neutrophilic granulocytes as a constituent of the normal CSF cytomorphology.

Miastenia grave é doença neuromuscular caracterizada por excessiva fatigabilidade da junção muscular e envolve, particularmente, músculos inervados por nervos cranianos. Acredita-se que o defeito esteja localizado na junção neuromuscular. Os autores estudaram os achados histoquímicos e e morfométricos em 4 pacientes com miastenia grave mostrando que as fibras do tipo II eram significativamente menores que as fibras do tipo I.

Myasthenia gravis is a neuromuscular disease characterized by excessive fatigability of muscle function and particularly involves muscles innervated by the cranial nerves. It is believed that the defect is in the neuromuscular junction. The authors studied histochemical and morphometric findings in 4 patients with myasthenia gravis showing that the type II fibres were significantly smaller than the type I fibres.

Three cases of complete basilar obliteration are reported, ocurring at ages arteriosclerosis is unusual. The obliteration was verified angiographically in these three cases. The evolution was quite different in each one: "restitutio ad integrum", death, and definitive persistency of a locked-in syndrome.

Abstract in Portuguese:

O caso descrito refere-se a paciente do sexo feminino, com 22 anos, com história de dois episódios de crise convulsiva lateralizada à direita, hemiparesia direita e disfasia. O exame neurológico mostrou ainda edema de papila bilateral. A tomografia computadorizada (TC) do crânio mostrou um cisto de 70x77 mm, ocupando a região fronto-parieto-temporal esquerda, ocasionando grande desvio da linha média, além de outras lesões menores, semelhantes, esparsas no parênquima. Com o diagnóstico de neurocisticercose foi submetida a craniotomia, sendo removidos o cisto maior (cisticerco racemoso) e pequeno cisticerco cortical (10x10 mm) para exame. Teve alta 13 dias após a cirurgia, com regressão completa do quadro neurológico, exceto o edema de papila (regressão parcial). Um mês após foi submetida a tratamento com praziquantel (50 mg/kg/dia durante 20 dias). O exame de LCR foi normal antes deste tratamento e, durante ele, apresentou reação celular eosinofílica, com positivação da reação de fixação de complemento para a cisticercose, ambas transitórias. A TC prévia ao tratamento mostrou várias áreas hipodensas de 10 a 40 mm, esparsas no parênquima, inclusive na área operada- A TC realizada três meses após mostrou diminuição do número de cistos do parênquima, persistindo apenas uma área hipodensa de 20 mm no local operado e dilatação do corno frontal do ven-trículo do mesmo lado. A evolução clínica foi excelente e o exame neurológico foi normal 3 meses depois.

Abstract in English:

The authors report a case of a female patient, age 22, who presented episodes of focal seizures, right hemiparesis and dysphasia of five months duration. The neurologic exam revealed bilateral papilledema and computarized tomography of the head showed a cystic lesion (70x77 mm) in the left fronto-temporo-parietal area, midline shift and similar lesions scattered in the brain parenchyma. She was submitted to a left craniotomy with the diagnosis of cerebral cysticercosis and the major cyst (Cysticercus racemosus) and a small cortical cyst (10x10 mm) were removed to aliviate the increased intracraneal pressure and for histopathological examination. Thirteen days after surgery she was discharged without neurological deficits, except mild papilledema. One month later she was treated with praziquantel (50 mg/kg/day) during 21 days. The CSF examination was normal before the treatment started and showed a transient eosinophilic cellular reaction with positivation of the complement fixation test for cysticercosis while on treatment. The CT-Scan performed before the medical treatment revealed ventricular dilatation and several low density areas (10 to 40 mm) scattered in the cerebral parenchyma, including the operative site. CT-Scan performed three months later showed small low density areas only in the left cerebral hemisphere, including the operative site, calcification in the parenchyma, and slight dilatation of the left lateral ventricle. The clinical evolution was uneventfull and the neurologic examination was normal three months later.

Relato de caso de doença de Parry-Romberg, com hemi-hipotrofia esquerda de predomínio facial, epilepsia, afasia amnéstica, rebaixamento intelectual, hemiparesia e hemianestesia direitas e sindrome cerebelar. É enfatizada a raridade da atrofia cerebelar (apenas um caso na bibliografia consultada) e a correlação entre as anormalidades clínicas e a tomografia axial computadorizada de crânio.

The authors report a case of Parry-Romberg disease with predominantly facial left atrophy, seizures, amnesic aphasia, mental retardation, right hemiplegia and hemianesthesia, and cerebellar syndrome. The rarity of cerebellar atrophy (only one case thus far reported in the literature surveyed) is emphasized, as well as the correlation between the clinical abnormalities and the computerized axial tomography of the skull.

Os autores relatam caso de paciente com 27 anos de idade com oftalmoplegia externa, anisocoria, ataxia, arreflexia profunda, disfagia, disfonia, paralisia facial periférica bilateral, hipopalestesia e discreta hipoestesia táctil palmo-plantar. O quadro teve início 10 dias após virose de vias aéreas superiores e ocorreu rápida melhora após terapêutica com prednisona. Os achados do exame neurológico e dos subsidiários são compatíveis a alterações centrais e periféricas.

Abstract in English:

The authors report a case of a 27 year old male patient who has exhibited external ophthalmoplegia, anisocoria, ataxia, absent deep reflexes, dysphagia, dysphonia, bilateral peripheral facial paralysis, hypopallesthesia and a mild sensory impairment of the four extremities. In addition, his cerebrospinal fluid showed albuminocytologic dissociation. The otoneurologic examination showed important findings. These symptoms began ten days after a "cold", and quickly subsided after prednisone therapy. Two months after the onset of clinical features, just a deep apendicular areflexia was detected on his neurological examination. The neurological signs and complementary laboratory findings suggest that both central and peripheral alterations are responsible for the clinical manifestations.

A síndrome de Déjérine-Roussy apesar de ter sido descrita desde 1900 esteve durante muitos anos longe da atenção dos neurologistas. Os autores apresentam um caso típico com lesões isquêmicas talâmica e do córtex calcarino oposto dando ênfase ao estudo por tomografia axial computadorizada bem como ao uso de novas drogas à base de vasodilatadores, anti-agregantes plaquetários e tiapride.

The Déjérine-Roussy syndrome was not revised for several years by neurologists, although it has been known since 1906. The authors describe a typical case involving thalamic ischemic and calcarine other side cortex lesions emphasizing its study by computed axial tomography and by using new drugs on a vasodilatation, inhibition of platelet aggregation and tiapride base.
Traumatic aneurysm of the superficial temporal artery: a case report Roca, Gustavo Rioja Picozzi, Luigi

Abstract in Portuguese:

Um caso de aneurisma traumático da artéria temporal superficial é apresentado. No Brasil apenas dois casos foram publicados. Na literatura, os ATTS são considerados consistentemente entidades cirúrgicas raras. Os autores sugerem que esta entidade talvez seja mais freqüente, alguns casos não sendo publicados ou passando desapercebidos.

Abstract in English:

A case of traumatic aneurysm of the superficial temporal artery is reported. In Brazil we have knowledge of only two published cases of ATTS. Review of the literature consistently indicates that this is a rare surgical entity. The authors suggest that perhaps ATTS is not so infrequent, some cases not being published or dismissed as unimportant.
Calcified cysticerci in the basal ganglia and Parkinson's syndrome: a case report Cavalcanti, Carlos Eduardo

Devido à raridade da neurocisticercose no Maranhão, que não é zona endêmica, assim como pela raridade do processo patológico observado, é feito registro do caso de paciente com 57 anos, maranhense, com parkinsonismo sintomático relacionado a calcificações nos gânglios basais por cisticercos, demonstrados mediante tomografia computadorizada.

Abstract in English:

It is reported the case of a 57 years-old man from Maranhão State who presented Parkinson's syndrome. Images proper to calcified cysticerci were found in the basal ganglia at the CT-scan examination. Considerations are made as to: the low frequency of cysticercosis in the northern region of the country where the patient lives; the association of symptomatic Parkinson's syndrome and cysticercosis of the central nervous system, which is rare; the demonstration of calcified cysticerci in basal ganglia in this situation, which was not registered up to the present time.
